Requirements

  • Graduate of an accredited nursing school with current licensure to practice in state of operation.
  • 1 year hospice nursing experience required
  • Excellent observation, verbal and written communication skills, critical thinking skills, basic math skills; nursing skills per competency checklist.
  • Knowledge of the hospice philosophy of care.
  • Commitment to clinical and documentation excellence.

Nice To Haves

  • 3+ years hospice nursing experience preferred

Responsibilities

  • Deliver compassionate, patient-centered hospice nursing care
  • Complete initial and ongoing comprehensive patient and family assessments
  • Develop, implement, and revise individualized plans of care
  • Administer medications and treatments as ordered by physicians
  • Monitor patient conditions and communicate changes to physicians and care teams
  • Educate and support patients and families regarding medications, symptom management, and care plans
  • Collaborate closely with the interdisciplinary hospice team to coordinate care
  • Assist patients with activities of daily living and promote comfort-focused care
  • Maintain accurate and timely clinical documentation
  • Serve as Case Manager when assigned and coordinate patient care for designated caseloads
  • Participate in interdisciplinary meetings, quality improvement initiatives, and ongoing training
  • Support onboarding and mentoring of newly hired nurses
  • Ensure compliance with all hospice policies, state regulations, and federal requirements
  • Participate in on-call rotation as defined by organizational policy
