As Regional Director of Patient Care Services, you'll oversee clinical operations across multiple home health service lines, guiding a team of Clinical Supervisors and ensuring patients receive safe, high-quality care in their homes. This is a hands-on leadership role—you'll shape how care is delivered, mentor the next generation of clinical managers, and drive the outcomes that matter most to patients and families.

Requirements

  • Active California RN or PT License
  • 3+ years of management experience in home health or a related healthcare setting (within the last 5 years), including at least 1 year in a supervisory or administrative role
  • Strong critical thinking, organization, and interpersonal skills
  • Valid California driver's license, auto insurance, and clean driving record

Responsibilities

  • Lead & Develop Your Team: Directly supervise 4–5 Clinical Supervisors across different service modalities. Coach, mentor, and grow leadership capabilities within your team. Conduct regular one-on-ones, performance reviews, and support accountability across branches.
  • Drive Clinical Excellence: Ensure care plans align with physician orders, agency policies, and patient needs. Step in on complex or escalated cases to guide clinical decision-making. Implement best practices and standardized care delivery models.
  • Own Quality & Compliance: Monitor key metrics: rehospitalization rates, patient satisfaction, timely start of care, OASIS accuracy, and star ratings. Keep the region survey-ready and lead corrective action when needed. Ensure compliance with federal/state regulations, Conditions of Participation, and infection prevention standards.
  • Optimize Operations: Balance staffing, scheduling, and caseloads to match census and acuity. Partner with leadership on productivity, resource allocation, and workflow efficiency. Support financial performance goals while protecting care quality.
  • Collaborate Across the Organization: Work closely with intake, quality, compliance, HR, and business development teams. Coordinate with physicians, hospitals, and community partners. Facilitate interdisciplinary care and communication.
