About The Position

The Regional Case Manager, Latino Outreach will provide culturally enhanced case management services for individuals with substance use or co-occurring disorders. This position will focus on recovery-oriented services that increase individuals access to equitable care, connect individuals to support services, improve individuals’ quality of life, decrease stigmas and promote health equity with a focus on Latino populations/groups. An emphasis will be placed on targeting culturally and linguistically competent services with a focus on addressing identified disparities in the global systems of care for Latino populations. The Regional Case Manager will collaborate extensively with outside service providers, and make appropriate internal/external referrals.

Requirements

  • Bachelor’s degree in relevant field and experience in case management are required.
  • The ability to understand more advanced computer skills and be proficient in the advanced knowledge of software such as Microsoft Word and Excel is also required
  • Experience in working with individuals facing addiction challenges.
  • Excellent interpersonal skills and the ability to maintain a positive collaborative relationship among staff
  • fluent in English and Spanish required.
  • Must possess a valid driver's license and maintain a driving record that meets company standards

Responsibilities

  • Provides community-based providers with information on substance abuse treatment and access to services
  • Ensures a region-wide integration and collaboration with other providers
  • Establishes linkages with other organizations within Region 2 which provide services to Latinos in order to facilitate referrals to such services
  • Facilitates and participates in monthly meetings with collaborators in order to share ideas, develop approaches review success and failures, in order to assist one another and plan for future directions
  • Works with the FHCHC data team to institute a tracking system that follows the individual through the various levels of the continuum of care
  • Works with the FHCHC data team to institute a tracking system to follow up on Latino individuals leaving treatment for non-compliance or against medical advice
  • Connects clients with needed services and supports including making internal and external referrals
  • Works closely with FHCHC’s Addiction Medicine Care Coordinator and other members of the FHCHC Care Coordination team, to help clients facing barriers such as housing, food insecurity, etc.
  • Ensures that all clients have the needed transportation to and from meetings/appointments related to withdrawal treatment, hazard reduction and recovery support. On some occasions, will have to transport the client himself/herself utilizing FHCHC-owned vehicle.
  • Maintains strict adherence to all document deadlines including report deadlines and timely completeness of documentation and documents client referrals, encounters and services in Epic (electronic health record)
  • Attends and actively participate in staff meetings, supervision, and trainings as directed and participates in community events relevant to assisting the Latino population with issues of substance use withdrawal treatment, hazard reduction, and recovery support
  • Will ensure that a minimum of 40 new patients per year enter into this Latino focused, substance use support program throughout the entire Region 2.
