About The Position

Big Bend Hospice is seeking a Referral Specialist to serve as the vital first connection between patients, families, healthcare providers, and our care teams. In this full-time role, you will help coordinate referrals for Hospice, Home Health, and Palliative Care services, ensuring patients receive timely, compassionate support. Valid Florida Driver's License, reliable transportation, and current auto insurance.
High School Diploma or GED required.
Minimum of two years of administrative or clerical experience required.

Nice To Haves

Associate degree preferred.
Experience in a healthcare or medical office setting preferred.
Knowledge of medical records and healthcare terminology preferred.
Active Florida Licensed Practical Nurse (LPN) license is a plus.

Responsibilities

Responding promptly and professionally to incoming referral calls.
Gathering and verifying patient, insurance, and referral information.
Coordinating admission visits with our clinical team.
Scheduling appointments and ensuring timely communication with patients, families, physicians, and referral sources.
Creating and maintaining accurate patient records within the electronic medical record (EMR) system.
Collaborating with Admissions Nurses, Social Workers, Finance, and Patient Care teams to facilitate smooth admissions and reimbursement processes.
Ordering equipment and supplies as requested to support patient care needs.
Communicating referral updates to internal teams and community partners.
Converting referrals to active admissions and maintaining accurate data throughout the process.
Supporting organizational goals while delivering compassionate, customer-centered service.
