About The Position
Behavioral Health Connections (BHC) in Dunwoody, ATL is seeking a dynamic and talented Referral Management Specialist Full Time 10 Hours Weekday Evening shift that serves to assess and coordinate the care of patients referred for Mental Health Services. They review referral information, determine the appropriateness of services, and facilitate the connection between patients and the most appropriate facilities or treatment programs.
Requirements
- Masters Degree in Social Work, Psychology, Counseling, Behavioral Health or related field.
- Current CPR, Vaccines, if required by a customer hospital.
- Must have a valid driver’s license.
Nice To Haves
- Preferred Licensed in state, LMSW, LCSW, LPC
- Preferred 1-3 years work related experience
Responsibilities
- Review assessment referrals received from various sources in collaboration with the RN and other clinicians, to assess patient needs.
- Assist in coordinating patient care.
- Responsible for service coordination by matching patients with appropriate mental health services and providers within UHS, considering factors such as specialty, location, systems complexities and insurance coverage.
- Maintain patient records, update databases, and ensure accurate documentation of patient information in approved EMR and other platforms.
