About The Position

We’re growing and looking for a Referral & Intake Nurse (LPN) to join our team and serve as a key connection point for patients, families, and referral partners across all Unity territories. This role is ideal for someone who thrives in a fast-paced environment, enjoys coordination and communication, and is passionate about supporting patients at the very start of their hospice journey.

Requirements

• Current unrestricted Licensed Practical Nurse licensure from the State of Wisconsin
• Valid WI Drivers’ License, reliable transportation, and proof of current automobile insurance coverage

Responsibilities

• Managing incoming referrals and gathering patient medical information
• Communicating with hospitals, clinics, and care teams
• Coordinating initial visits with patients, families, and admissions staff
• Verifying insurance coverage and obtaining necessary orders
• Supporting eligibility review and following up on pending referrals
• Maintaining accurate and timely documentation
