About The Position

FamilyCare Health Centers is looking for a Referral Coordinator at our St. Albans Location. This role involves assisting providers in offering comprehensive health care services by coordinating patient care with other health care professionals and facilities. The coordinator will arrange patient care that may include x-ray, imaging, procedures, laboratory tests, specialty and sub-specialty care, counseling, home health, or other supportive services provided outside of FamilyCare. A key responsibility is ensuring all referral orders are tracked and tied to corresponding reports for complete patient records. The role also requires communicating referral information to patients, specialists, office staff, providers, and payer sources, and maintaining accurate patient health records.

Requirements

• High School graduate or equivalent
• Experience in computer data entry
• Knowledge of medical terminology

Responsibilities

• Assist providers in offering comprehensive health care services for our patient by coordinating the patient's care with other health care professionals and facilities.
• Arrange patient care that may include x-ray, imaging, procedures, laboratory tests, specialty and sub-specialty care, counseling, home health or other supportive services, that must be provided at a facility outside of FamilyCare.
• Ensure that all referral orders will be tracked and tied to the corresponding report when it is received to ensure that FamilyCare obtains complete information for the patient record.
• Communicate referral information to patients, specialist, office staff, providers, and payer sources and keeps patients and their health record accurately updated.
