About The Position

Referral Coordinator About the Role Are you someone who thrives on organization, communication, and helping patients navigate the healthcare system with confidence? Thundermist Health Center is seeking a Referral Coordinator to support patients and providers by ensuring referrals are processed accurately, efficiently, and compassionately. In this role, you’ll serve as a key connection point between patients, providers, specialists, and insurance entities to help coordinate timely access to care. The ideal candidate is detail-oriented, customer-service driven, and comfortable managing multiple priorities in a fast-paced healthcare environment.

Requirements

  • High school diploma or equivalent
  • Strong organizational and multitasking skills
  • Intermediate to advanced computer proficiency
  • Excellent customer service and telephone communication skills
  • Ability to work independently and collaboratively within a team environment

Nice To Haves

  • Post-secondary training through a Medical Assistant or professional healthcare program
  • Bilingual in English and Spanish
  • Previous experience working with medical referrals or care coordination

Responsibilities

  • Process urgent and routine referrals within established timeframes
  • Coordinate appointments with specialists and external facilities
  • Educate patients on referral timelines, expectations, and next steps
  • Communicate with specialty offices to obtain consult notes and follow up on pending referrals
  • Monitor referral work queues and perform patient outreach to address barriers to completion
  • Obtain insurance authorizations and ensure all required documentation is submitted accurately
  • Access external provider portals to retrieve and update patient records
  • Respond promptly to incoming calls, voicemails, and patient inquiries
  • Maintain accurate, concise documentation within the electronic medical record system
  • Collaborate with internal teams and external providers to support continuity of care
