About The Position

The Referral Coordinator I set appointments and process transfers to ensure timely transmission and start of services. Primarily connected with the healthcare industry, Referral Coordinator I serves as a liaison for patient resources. This position must be executed in compliance with insurance requirements, and the coordinator will often work closely with insurance companies. The Referral Coordinator I also performs general office duties that requires proficiency in communication and organizational skills. Specific understanding of the field in which they work, including office procedures and liability considerations, are necessary to social services, or learning institutions.

Requirements

  • High School Diploma or GED
  • Certificate of Completion for a Medical Assistant or Medical Administrative Program
  • Ability to conduct oneself in a profession manner
  • Ability to cooperative and collaborate with a team of healthcare professional to provide non-discriminatory care to underserved patient populations
  • Ability to use Word, Excel, Power point, and Outlook
  • Ability to learn and use Electronic Health Record Systems (e.g., IMS, EPIC and Dentrix).
  • Ability to manage multiple tasks in a high volume environment.
  • Ability to prioritize and perform a variety of tasks in a fast paced environment.
  • Ability to take initiative and function with a high level of independence and problem solve.
  • Ability to maintain a good attendance and report to work on time
  • Ability to effectively communicate (oral and written)
  • Ability to provide good customer/patient services
  • Ability to adhere to the health center’s policies and procedures include but not limited to safety, employment and care services.

Responsibilities

  • Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety.
  • Ensure complete and accurate registration, including patient demographic and current insurance information.
  • Assemble information concerning patient’s clinical background and referral needs to provide appropriate clinical information to specialist.
  • Contact review organizations and insurance companies to ensure prior approval requirements are met and provide necessary medical information such as history, diagnosis and prognosis.
  • Provide specific medical information to financial services to maximize reimbursement to hospital and physicians.
  • Review details and expectations about the referrals with patients.
  • Assist patients in problem solving potential issues related to the health care system and families or social barriers (e.g. request interpreters as appropriate, transportation services or prescription assistance).
  • Be the system navigator and point of contact for patients and families with questions and concerns.
  • Assume an advocate role on the patient’s behalf with the carrier to ensure approval of the necessary supplies/services for the patient in a timely fashion.
  • Identify and utilize cultural and community resources to establish and maintain relationships with identified service providers.
  • Ensure that referrals are addressed in a timely manner.
  • Remind patients of scheduled appointments via mail or phone.
  • Ensure that patients’ primary care chart is up to date with information on specialists, consults, hospitalizations, ER visits and community organization related to their health.
  • Adhere to HIPAA, OSHA, other governing authority regulations, and Internal Policies and Procedures
  • At all times you will perform and undertake such other duties and responsibilities as are requested of you by Management
