The Reentry Support Specialist provides person-centered support to individuals transitioning from incarceration, helping them address behavioral health, housing, employment, and other reentry-related needs. This full-time position is responsible for conducting assessments, developing Individualized Service Plans (ISPs), delivering direct services, and connecting participants to critical community resources. Working within the EXIT-318 Reentry Program, the Specialist collaborates with internal staff and external partners to promote stability, reduce recidivism, and ensure compliance with program goals and reporting requirements. The ideal candidate is compassionate, detail-oriented, and committed to empowering justice-involved individuals through holistic, solution-focused support.
