About The Position

Community Health and Wellness, LLC is seeking Certified Therapeutic Recreation Specialists to provide services for individuals with disabilities throughout Texas. This role involves one-on-one home and community Recreation Therapy for children and adolescents with intellectual disabilities, physical disabilities, and behavioral/mental health diagnoses. The position requires travel to client homes and facilities, and the ability to transport clients during visits. Candidates must be certified and possess reliable transportation and a valid driver's license.

Requirements

CTRS or TRS/TXC certified
Minimum of Bachelors Level Degree required in Therapeutic Recreation, Recreation Administration with emphasis in Therapeutic Recreation, Leisure Studies with emphasis in Therapeutic Recreation, and/or related degree to Therapeutic Recreation.
CPR/AED/FIRST AID Certification (American Red Cross or American Heart Association only)
Reliable transportation
Valid driver's license
Agency-required auto liability insurance (100k/300k Bodily Injury)
Access to a private personal laptop or desktop computer for HIPAA compliance
Professional Liability Insurance (with $1 million/$3 million coverage limit)
Social Security Card

Responsibilities

Provide one-on-one home and community Recreation Therapy to children and adolescents with intellectual disabilities, physical disabilities, and behavioral/mental health diagnoses.
Create tailored goals to meet the needs of individual clients.
Document on each session.
Communicate with other team members and service providers.
Turn in timely documentation including weekly invoicing, progress notes, and related paperwork.
Travel to current or prospective clients’ homes.
Transport client(s) during client visits.
Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader.
Implement patient care plans through home and community goal-related activities.
Work independently, manage one’s own time and the time of client(s).
