The Recovery Support Navigator is responsible for providing quality services and case management to assigned clients in accordance with the Recovery Navigator model. This role involves performing intake assessments on all new clients to the program and completing all associated documentation within required time frames. The Navigator works with clients to develop and implement individualized plans with goals and objectives based on identified needs. Additionally, the position assists clients with scheduling and coordinating medical and behavioral health appointments.
