About The Position

HealthRIGHT 360 is a nonprofit organization dedicated to providing accessible healthcare to vulnerable populations, aiming to overcome systemic barriers and promote health equity. They offer a range of services including mental health care, substance use treatment, and primary health services, along with transitional support for individuals re-entering the community. Their Treasure Island Residential Step Down (RSD) program offers a temporary, drug and alcohol-free environment for residents actively engaged in outpatient substance use disorder treatment. The On-Call Recovery Coach plays a crucial role in assessing client needs and strengths, providing individual coaching and case management to help clients navigate systems of care and maintain a supportive recovery environment. This role also involves ensuring the safety and security of the program and its residents through regular house runs and effective communication with the interdisciplinary team, including crisis intervention when necessary.

Requirements

  • High School diploma or equivalent.
  • First Aid and CPR certification.
  • A valid California driver’s license and automobile insurance.
  • Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency.

Nice To Haves

  • A Drug and Alcohol Registration/Certification recognized by DHCS, is preferred.
  • Bachelor’s degree in related field is preferred.
  • Preferably bilingual in San Francisco threshold languages.
  • Knowledge of co-occurring disorders and trauma informed treatment.
  • Knowledge of Clinical documentation (treatment plans, progress notes etc.).
  • Knowledge of gender-responsive, trauma informed, and co-occurring treatment.

Responsibilities

  • Welcome and assist clients with navigating the building and provide excellent customer service.
  • Assist patients with required paperwork for enrollment in services.
  • Coordinate transportation for patients in residential programs.
  • Provide direct services such as intakes, assessment, crisis intervention, and case management.
  • Facilitate individual coaching sessions with each caseload client to support retention and completion.
  • Provide learning experience opportunities and support to assist clients in meeting long-term recovery goals.
  • Proactively link clients to internal and external resources based on recovery needs and follow up on progress.
  • Perform house runs to ensure client accountability and facility safety and security.
  • Perform crisis intervention and communicate with the treatment team.
  • Document client updates and incidents in the facility log daily.
  • Perform CPR and First Aid, including administering Narcan as needed.
  • Work weekends and holidays as needed.
  • Facilitate community meetings and ensure compliance with facility rules.
  • Document all client updates and facility incidents daily in the facility log.
  • Perform periodic house runs to ensure facility safety and security.
  • When assigned at the Health and Wellness office, document and accurately distribute client money, ensure client medications are securely stored and accounted for, and hold facility keys and floater cellular phones.
  • Accompany clients to offsite appointments related to their recovery as needed.
  • Attend all required trainings and meetings.
  • Plan and facilitate client celebrations and special events.
  • Collaborate with clients and resources to develop transition plans, progress notes, and updates.
  • Properly document all individual encounters and complete discharge paperwork and required agency assessments in a timely manner.
  • Maintain accurate records by data entering documentation into electronic systems.
  • Complete accountability reports weekly and collaborate with treatment providers to assist clients in achieving program completion.
  • Remain alert and awake during the entire shift.
  • Ensure and maintain a safe and orderly environment for clients, staff, and visitors.
  • Direct and deflect traffic; de-escalate tense situations; and foster a safe, respectful, and welcoming environment.
  • Maintain positive, respectful, non-confrontational behaviors.
  • Make regular rounds to monitor entrances, exits, offices, common areas, and building perimeters.
  • Ensure a welcoming environment by clearing areas of crowds, debris, and out-of-place items.
  • Work collaboratively with service providers and administrative staff to maintain an orderly environment.
  • Perform CPR and First Aid as needed.
  • Perform other duties as assigned.
