About The Position

High Point & Affiliated Organizations (HPAO) is a health and human service agency dedicated to treating and preventing substance use disorders and mental illness. With programs across Southeastern Massachusetts, High Point offers a comprehensive continuum of care, including inpatient, outpatient, residential, and community-based services. The agency also supports survivors of abuse, violence, and families experiencing homelessness. Requirements

Present as courteous and professional at all times.
Demonstrated flexibility and ability to perform multiple tasks.
Excellent communication skills, including the ability to organize ideas in a logical and clear fashion.
Knowledge of basic computer skills and software programs, including the ability to learn, use, and train HPAO Electronic Health Record (EHR).
Ability to work cooperatively and collaboratively with others.
Understanding of the relationship between substance use and mental health issues and/or self-help programs.
Understand and maintain therapeutic boundaries and reinforce rules respectfully.
Comply with Federal Regulations, 42 CFR Part 2, Confidentiality of Substance Use Disorder and HIPAA Regulations.
TB/Flu and Covid19 Assessment.
Completion of the HPAO approved de-escalation program.
Successful completion of all trainings upon hire and annually as required.
One-year experience in substance use treatment and/or mental health field.
Personal lived experience in mental health and/or substance use recovery required.

Nice To Haves

High School Diploma or GED, preferred.
Associate's degree in human services, preferred.
CPR Certification, preferred.

Responsibilities

Maintain assigned caseload.
Provide direct care to residents, including assessments, individual counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily group facilitation, group note completion, and bio-psychosocial assessments.
Make referrals when appropriate.
Manage assigned residents' medical records, ensuring all entries are professional, complete, thorough, and timely.
Link clients with aftercare services prior to discharge and assist residents and their families in accessing services.
Strengthen existing and create new community partnerships to help clients develop skills for sustaining sobriety and/or housing.
Review patients'/family rights and responsibilities, emergency safety plans, and grievance policy.
Communicate and involve family in treatment at the patient/client/resident's request.
Maintain professionalism when connecting with individual's collaterals.
Attend and participate in all unit, facility meetings, case conferences, Treatment Team meetings, and mandatory trainings as directed by the Clinical Supervisor/Director.
Engage in continued education in the field respective to their service level.
Engage with patients in community settings and inpatient units to offer services.
Complete initiation for services and all required documentation based upon services agreed upon.
Provide ongoing monitoring and compliance with required documentation for services.
Monitor service authorizations and provide updated information for continued stay reviews.
Perform care coordination activities to ensure referrals for services are completed.
Monitor consents & releases upon admission and as needed thereafter.
Assist with scheduling appointments for follow-up with designated community providers.
Participate in program initiatives, quality projects, and program support services.
Engage in training opportunities to enhance knowledge of mental health and SUD services.
Engage and attend community events with stakeholders, agencies, and service providers.
Engage patients with their individualized recovery plan.
Support community-based needs including appointments, attending self-help meetings, and accessing community resources (e.g., housing, SNAP/WIC).
Complete Recovery Coach Academy and obtain Recovery Coach Certification within 6 months of hire.
Perform other duties as assigned by Program Director / Clinical Supervisor/ COO.
