Requirements

High school diploma or GED.
At least one (1) year of experience in medical or mental health or a related area.
CCAR (Connecticut Community for Addiction Recovery) Certification or Advocacy Unlimited Certification required.
Experiential or actual knowledge of Medication Assisted Treatment (MAT).

Responsibilities

Provides recovery services to clients based on their needs and preferences, serving as a 'recovery guide' and role model.
Re-engages patients who meet program criteria, acting as a liaison with the counselor and supporting the patient in completing community referrals.
Maintains a caseload of Intensive Case Management clients and assists them in various recovery life skills (personal care, independent living, health & safety, money management, transportation, interpersonal communications).
Monitors client progress in completing recovery goals.
Coordinates transportation for patients to necessary community services.
Attends community meetings to market the program and develop community resources.
Advocates to ensure patients have access to and receive services from community-based programs.
