About The Position

The referral and entry coordinator plays a key role in facilitating the referral process. This position supports the assistant program director by evaluating the eligibility determination of individuals served by the REACH program. Primary duties include receiving and processing referrals, requesting, evaluating and maintaining individual records, providing program overview to referral sources, establishing rapport with individuals, families, and systems within catchment area, and maintenance of individual services database. This position also supports the mobile crisis team by participation in on call duties as needed.

Requirements

  • Licensed or Licensed-Eligible as a LCSW, LPC, LMFT
  • Experience in intellectual/developmental disabilities, including programming for individuals with significant behavioral needs.
  • Knowledge of the MH and I/DD systems of care.
  • Knowledge and ability to utilize computer word processing and data programs including but not limited to Microsoft Office Suite, Web-based data entry, etc.
  • Must possess a valid, unrestricted driver’s license appropriate to state requirements and be able to operate a motor vehicle safely for work-related travel.

Responsibilities

  • Implements REACH supports for individuals with intellectual/developmental disabilities and co-occurring mental illness or behavioral health needs and those persons over 18 years of age who are not formally diagnosed with an I/DD Diagnosis
  • Collaborates with Clinical Director to evaluate diagnostic criteria and determine client eligibility in accordance with program standards.
  • Creates and maintains spreadsheets and other data tools for data collection, reporting purposes, and case assignments.
  • Facilitates opening/closure processes of REACH clients in electronic health record, data reporting system, internal tracking system, and client notification.
  • Maintains linkages and relationships with community partners.
  • Participates in daily triage meetings and other required meetings with Team Members, REACH Director, Assistant Director, CTH staff and other applicable parties.
  • Collaborates with Case Managers, Clinical Home Providers, CSB representatives and/or applicable crisis responder (Emergency Services, Mobile Crisis Teams, Law Enforcement, Hospital Emergency Departments, Walk-in Crisis, etc.).
  • Collects and enters required data and produces documentation on administrative and service delivery and overall utilization of REACH services within established timeframes and according to set protocols.
  • Participates in mobile crisis on call rotation as needed.
  • Other duties as assigned/required.
