About The Position

The Reentry Coordinator develops, maintains, and tracks community partnerships and referral relationships to support successful patient transitions from correctional facilities to community-based care. This position collaborates with medical, behavioral health, case management, and community providers to ensure continuity of care and access to essential healthcare and support services following release.

Requirements

  • Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or a related field required.
  • Current BLS certification through the American Heart Association or American Red Cross required.
  • Subject to initial and ongoing security clearance requirements.
  • Strong communication, organizational, and problem-solving skills.
  • Ability to build collaborative relationships with internal and external stakeholders.

Nice To Haves

  • Master's degree preferred.
  • Experience in case management, discharge planning, behavioral health, social services, or healthcare preferred.
  • Correctional healthcare or reentry services experience preferred.

Responsibilities

  • Develop and maintain referral relationships with community-based organizations, primary care providers, behavioral health agencies, developmental disability programs, substance use treatment providers, crisis response teams, and other community resources.
  • Collaborate with medical, behavioral health, and case management staff to facilitate discharge planning and successful patient reintegration into the community.
  • Conduct needs assessments to identify barriers to care and available community resources.
  • Develop, maintain, and update databases of community healthcare providers and support organizations.
  • Coordinate and ensure completion of all required Release of Information forms and related documentation.
  • Document patient interactions, discharge planning activities, referrals, and progress notes in approved electronic health record systems.
  • Monitor discharge planning outcomes and recommend strategies to improve continuity of care and reduce recidivism.
  • Ensure compliance with HIPAA, NCCHC, ACA, and all applicable state and federal regulations.
  • Develop and participate in quality improvement initiatives focused on access to care and reentry services.
  • Participate in interdisciplinary meetings, committees, and case reviews as required.
  • Follow all infection control and personal protective equipment (PPE) requirements.
  • Perform other duties as assigned.
