About The Position

Trillium Health Resources is seeking a Re-Entry Care Coordinator to support individuals transitioning between care settings who are justice-involved and preparing for release from Department of Corrections or county detention facilities. This role provides care coordination and transition planning services, develops transition plans, aligns services across providers and community partners, and addresses barriers to support successful and stable transitions. The work changes lives by helping individuals, including children and adults with severe mental illness, thrive and live independently.

Requirements

  • Must meet Qualified Professional (QP) status per NC requirements through one of the following, with at least one (1) year of experience with the forensic population (working with individuals involved in the criminal justice system).
  • Fully licensed by the NC governing board regulating Human Services professions; OR
  • Fully licensed RN who is licensed to practice in the state of NC by the NC Board of Nursing who also has four (4) years of full-time experience with the MH/DD/SA population served.
  • A graduate of a college or university with a Master’s degree in a human service field and has one year of full-time, pre or post-graduate degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has one year of full-time, pre or postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling.
  • A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has two years of full-time, pre or post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
  • A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has four years of full-time, pre or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
  • Must have a valid driver’s license.
  • Must reside in North Carolina and within 75 miles of New Hanover County.
  • Must be able to travel within catchment as required.

Responsibilities

  • Coordinate and manage care transition activities for recipients on the High Priority Re-entry list to support successful discharges and service changes.
  • Develop and implement transition plans based on individual needs and transition requirements.
  • Collaborate with providers and partners to align services during transition periods.
  • Monitor transitions and intervene to resolve delays or barriers.
  • Facilitate service connections and ensure continuity following transitions.
