About The Position

The incumbent serves as the Re-Entry Coordinator for the Siletz Tribal Reentry and Management (STREAM) Program. This position will also include responsibilities related to the deflection program development as CTSI expands pathways for early intervention and law enforcement assisted diversion. The incumbent shall be sensitive to the diversity of Indian cultures as well as have knowledge of the disease of alcoholism/substance use disorders and recidivism. The incumbent must be sensitive to gender specific and gender diversity needs and issue. Work hours will be during the standard operating hours of CTSI Monday thru Friday 8:00am to 4:30pm and may include some evening and weekend hours.

Requirements

High School Diploma or GED with two years’ experience in a human service – related capacity, OR – an equivalent combination of education and/or experience.
Must become certified within 18-months as a CADC I through the Mental Health & Addiction Certification Board of Oregon (MHACBO). Upon successful completion of certification as a CADC I, incumbent will increase to a TL 119.1.
Knowledge of group dynamics and demonstrated ability to facilitate and coordinate curriculum-based groups.
Knowledge of substance-related disorders including client evaluation, criminality, and client case management, program policies and procedures, record keeping, and Documentation of services.
Knowledge of the Oregon and federal criminal justice systems.
Knowledge of Tribal and other community resources including housing, self-sufficiency, culture, employment, mental health, and residential treatment facilities.
Basic understanding of trauma informed care with the ability to identify and refer.
Experience in planning and budgeting, data collection and reporting.
Experience in managing budgets and coordinating services to maximize financial resources.
Experience in collaboration with other agencies and developing partnerships for service delivery.
Experience in working with justice-involved populations and/or supporting partnerships between behavioral health programs, courts, probation/parole, and law enforcement for deflection pathways.
Ability to work with a wide variety of people, and communicate effectively with program staff, clients, community partners and collaborating agencies.
Ability to demonstrate cultural competency in direct service, within the organization, and in program planning and development.
Ability to network effectively with Tribal programs community agencies and the multidisciplinary team including law enforcement and judicial partners involved in deflection initiatives.
Ability to become licensed as a CADC I through MHACBO within 18 months of hire and maintain certification while in this position.
Ability to plan, budget, perform data collection, and reporting for Re-Entry.
Ability to provide in-person contacts with potential clients who are incarcerated.
Ability to collaborate with prison/jail staff while planning for successful re-entry to the community.
Ability to collaborate with law enforcement and parole/probation staff for successful completion of requirements.
Must be able to pass a mandatory drug screening and have an acceptable background investigation.
Must have first Aid and CPR or obtain within 90-days of hire.
Must be able to work a varied schedule, which may include some evening and weekend hours.
Must possess a valid Oregon State driver's license, insurance and be eligible to drive a GSA vehicle.
Must be able to adhere to confidentiality of information as defined in the CTSI Personnel Manual and federal law.
Must adhere to state and federal child abuse reporting laws.

Responsibilities

Conduct interviews, evaluations, and formulate diagnostic impressions when licensed as a CADCI.
Prioritize treatment plan objectives including services, activities, and family participation, by communicating with primary counselors, documenting progress, identify obstacles, participate with clinical team addressing ASAM Placement Criteria levels of care.
Establish and maintain up-to-date, accurate client case records and submit activity and service records. Enter client information into NextGen or other electronic health records and/or state database.
Support mental, physical, and recreational program activities on the ISP including skill building for independent living, and referrals to self-help groups and other service delivery.
Work with the Peer Support Mentors to assist clients in carrying out the individualized plan.
Make referrals to services and other community resources as appropriate.
Advocate for clients in accessing services while addressing social determinants of health.
Assist individuals in aftercare planning and service delivery.
Be a team member; assist in the continued development and implementation of an overall comprehensive behavioral health program.
To the extent of resources available and as clinically indicated, shall provide clients with information regarding prevention; education and referrals to community and tribal services; and various private, civic, local or governmental agencies/groups.
Communicate and coordinate with Lincoln County Treatment courts to maximize services for participants.
Continuously improve the business processes used in the department and the electronic data system so that it meets the needs of the department to ensure accurate health care service reporting and billing.
Refer to resources that provide skill building for independent living, physical, and recreational activities and barriers to self-sufficiency.
Maintain up-to-date client records according to Indian Health Service, state, and program standards.
Participate in weekly clinical staffing meetings.
Collect, observe, and enter UAs as needed.
Interview and provide intakes for individuals applying for services.
Prepare and complete program reports, including input of data into tribal and/or state software systems, as appropriate.
Communicate and coordinate with State and Local correctional services, including Oregon Department of Corrections and Parole and Probation Offices, for the purpose of care coordination, referrals, and transition planning for clients enrolled in the Re-Entry Program.
Assist in efforts to secure additional funding.

Coordinate and manage the Tribal Deflection Program by serving as the primary point of contact for law enforcement, courts, parole/probation, and community partners, while responding to referrals and ensuring timely screening, triage, and connection to appropriate treatment and support services as an alternative to arrest or citation.
Collaborate with the Siletz Tribal Behavioral Health team and local partners to support coordinated care and develop Tribal-specific deflection pathways.
Help design and improve deflection processes and policies, track program data and reporting requirements, and identify service gaps to support ongoing program improvement.
Ensure services are culturally responsive and grounded in Siletz values by connecting Tribal members to cultural resources, community supports, and recovery services while building trust through respectful, relationship-based engagement.
Maintain accurate documentation, participate in team and coordination meetings, and support on-call Deflection Line coverage as needed.
Support diversion from incarceration, enhance community safety through a focus on healing and recovery, and help Tribal members navigate complex systems with dignity and support.
