About The Position

The Rapid Response Caregiver is a member of the home care team who works under the supervision of a Registered Nurse (RN) and performs various personal care services as called upon to meet the client’s needs. The Rapid Response Caregiver is dispatched as needed to a variety of clients due to last-minute cancellations, caregiver warm handoff/training needs, caregiver shadowing, and start of care for new clients. They are responsible for observing clients and reporting/documenting the observations and care performed. The Rapid Response Caregiver will be assigned in a manner that promotes quality, continuity, and safety of a client’s care. The Rapid Response Caregiver performs various personal care services as necessary to meet the client’s needs and serve as the Ambassador of AccuCare. The Rapid Response Caregiver is dispatched immediately to ensure continuity of client’s care. The Rapid Response Caregiver must consistently demonstrate and foster the following core values behaviors during all interactions with owner, clients, employees, vendors, and other business partners. The Rapid Response Caregiver must demonstrate and execute the following position-specific competencies to ensure effective, profitable, and patient-centered care.

Requirements

  • High school diploma or GED
  • Minimum of three (3) years of documented experience required in health care industry
  • Minimum of one (1) year of documented experience in home health and/or private duty agency.
  • Certified in CPR
  • Negative TB skin test or chest x-ray
  • Valid driver’s license and verifiable auto insurance
  • Ability to be calm in stressful situations
  • Self-directed; able to work successfully with little direct supervision; equally able to work successfully as a team member
  • Ability to interact with adverse, and sometimes difficult clients and/or family
  • Familiarity of St. Louis County and ability to travel within AccuCare service area.

Responsibilities

  • Dispatched to ensure continuity of client care due to last minute caregiver cancellations and/or start of care of newly acquired client.
  • Provide training/mentoring of caregivers through warm handoff introductions.
  • Shadow caregivers for complete, in-depth training of client’s habits, routines, and care needs as outlined in care plan.
  • Actively participate in process improvement initiatives of care plans, caregiver training, client care needs, etc.
  • Provide personal care tasks, as outlined in Care Plan, including bathing, grooming, dressing, toileting, feeding, taking and recording vital signs, measuring and recording height and weight, supporting good skin care, assisting with active and passive range of motions, providing help walking and transferring.
  • Observe client’s physical, mental, and emotional conditions and report any change to the Director of Nursing or nursing supervisor, and document observed changes.
  • Measure and document intake and output, if necessary.
  • Assist client in the self-administration of medication (medication reminder).
  • Perform light housekeeping.
  • Perform meal preparation and clean up.
  • Change bed linens and make up the client’s bed.
  • Maintain an orderly client environment.
  • Provide transport to doctor appointments, outside activities and shopping.
  • Participates in client teaching according to the client’s plan of care.
  • Follow principles of infection control and Universal Precautions.
  • Assist in providing a safe environment and promote quality patient care through adherence to established policies, procedures, and standards.
  • Ability to interact professionally with diverse, sometimes, difficult clients/family.
  • Ability to maintain a friendly, calm demeanor.
  • Express empathy for the needs of the client.
  • Treats clients, staff and the public with courtesy, respect and presents a positive public image.
  • Scheduled Rapid Response Hours: Represent the guaranteed hours as agreed upon.
  • When scheduled for a Rapid Response shift, our scheduling team will contact you as shifts become available.
  • Multiple shifts may be assigned in a single day, up until your scheduled end time.
  • If no Rapid Response shifts are assigned, the scheduled hours will be paid as Paid Non-Worked (at established rate, any hours worked during Rapid Response hours, will be paid accordingly).
  • If you work fewer hours than scheduled, AccuCare will make up the difference (ie Guaranteed amount of hours).
  • If you decline or fail to respond to a Rapid Response shift offer, you forfeit guaranteed hours for that day.
  • Any hours worked outside Rapid Response shifts are not included in the guaranteed hours or at Rapid Response pay rate.
  • Ensure client’s rights are adhered to, including the right to privacy and dignity, the right to accept or refuse care, and the right to confidentiality of their information.
  • Ensures confidentiality and security of the client’s medical information.
  • Use equipment properly and in a safe manner.
