About The Position

Cornerstone Family Healthcare is actively recruiting for a RAP Intensive Specialist to join our growing team in Middletown, NY. This role provides comprehensive medical case management services to Cornerstone Family Healthcare patients living with HIV (PLWH). Requirements

  • Must be Bilingual (English/Spanish)
  • Bachelor of Science/Art in Social Work, Psychology, Human Services or related field.
  • Highly organized with excellent oral and written communication skills.
  • Ability to maintain a nonjudgmental disposition and communicate with a diverse population.

Responsibilities

  • Provide comprehensive case management services to PLWH.
  • Provides a client-centered service that links patients with health care, psychological and social services in a manner that ensures timely, coordinated access to medically appropriate levels of care and support services and continuity of care.
  • Develops linkages and networks with other groups and agencies offering services to PLWH to help promote cooperation and reduce duplication of services among agencies.
  • Establishes peer support groups and focus groups for our patients, and PLWH referred from other agencies.
  • Provide crisis intervention to patients.
  • Provides follow-up and outreach for those patients not engaged in care.
  • Provide documentation to reflect patient interaction, outreach attempts service plan updates, and biannual psychosocial assessments.
  • Performs updates and maintenance of patient charts.
  • Provides case management input and psychosocial information concerning patients during medical conferences.
  • Enter case management data into EMR and AIRS databases.
  • Attends conferences and various training sessions, as needed.
  • Refers patients to programs, support groups and medical treatment, as needed.
  • Develop relationships and represent Cornerstone Family Healthcare in community organizations.
  • Attend and participate in monthly department, All Staff and other required meetings.
  • Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the Cornerstone Family Healthcare Policy and Procedure Manual and Employee Handbook.
  • Maintain confidentiality of all aspects of Cornerstone Family Healthcare including, but not limited to, patient confidentiality, financials, and employee relations.
  • Perform other duties as assigned.
