About The Position

The RAI & Restorative Lead Coordinator is a Full Time position reporting to the Director of Care. This role is responsible for the review of interdisciplinary and comprehensive resident assessments, collection of data for quality documentation management, and adherence to the Minimum Data Set (MDS) and InterRAI LTCF requirements. The position also provides support to the nursing and accounting departments.

Requirements

  • 3-5 years experience in Long Term Care
  • Current registration as RPN in good standing with the College of Nurses is required.
  • Current InterRAI certification is required.
  • Current Relias certification is required.
  • Must be able to maintain complete confidentiality of information.
  • Knowledge and experience with adult education strategies/facilitation.
  • Strong analytical skills; knowledge in trending and analysing data.
  • Well-developed oral and written (legible) communication skills.
  • Proficient computer skills and strong problem-solving abilities with excellent time management.
  • Clear aptitude for detail-oriented work.
  • Strong understanding of Point Click Care and RAI-MDS.
  • Excellent organizational, communication and interpersonal skills.
  • Must be flexible to work variable shifts, self-directed and able to function in a fast-paced environment.
  • Candidates will be required to provide a Vulnerable Persons Sector Police Check (acceptable if within 6 months).

Nice To Haves

Responsibilities

  • Managing the co-ordination and completion of the Resident Assessment, Clinical Assessment Protocols and care plans as assigned.
  • Complying with MLTC regulations regarding completion of the InterRAI LTCF process.
  • Completing accurate coding of the InterRAI and care plan documentation for all residents.
  • Continually updating knowledge base related to data entry and computer technology.
  • Chairing RAP chat as required.
  • Providing restorative education, support and assistance to staff and residents.
  • Assessing each new resident in regard to restorative needs.
  • Developing and documenting restorative goals and individualized plan of care for resident.
  • Re-evaluating care plan as per restorative care policy and procedure.
  • Monitoring and training staff regarding restorative tasks and resident specific goals.
  • Recording statistics and keeping records as required activities.
  • Coordinating Bed Entrapment Prevention Program.
  • Using supplies and equipment as directed and providing input in equipment selection.
  • Attending care conferences as required and participating in Quality Management.
  • Participating in the quality management program and completing restorative audits as required.
  • Charing restorative committee.
  • Participating in setting realistic and achievable departmental goals.
  • Completing and ensuring residents’ care assessments, service plans and documentation are up to date, and changes communicated to DOC for billings.
  • Assisting with scheduling of residents’ care conferences and participating in conferences.
  • Facilitating and providing resident/family education.
  • Coordinating nursing team’s resident care assignments to ensure efficient and timely delivery of services to residents.
  • Responding to and managing significant changes in residents’ condition and or medical emergencies.
  • Responding to resident/family concerns and ensuring appropriate action taken and or concern communicated to DOC/Executive Director and other departments.
  • Assisting the DOC in the coordination, implementation and monitoring of Infection Prevention and Control, Health and Safety and Clinical Programs and Quality Plans for the Nursing Department.
