The Contra Costa County Health Services Department is seeking qualified individuals for four (4) vacant Quality Management Program Coordinator positions. Two (2) positions are located at Contra Costa Regional Medical Center (CCRMC) and two (2) are located at Contra Costa Health Plan (CCHP), all positions are located in Martinez, California. When assigned to Contra Costa Regional Medical Center: The Quality Management Program Coordinator positions will be assigned to the Quality Department at CCRMC. These positions support the organization's commitment to delivering safe, high-quality, equitable care across health system settings, including acute and ambulatory care environments. These positions lead complex quality management, performance improvement, patient safety, accreditation, regulatory compliance, and strategic initiative efforts that support organizational goals and regulatory requirements. Incumbents provide program management and oversight for quality-related initiatives, working collaboratively with clinical, operational, and administrative leaders to improve patient outcomes, enhance care processes, strengthen patient experience, and ensure compliance with federal, state, and accrediting agency standards. The Quality Department is a team of professionals dedicated to improving quality of care, patient safety, regulatory compliance, accreditation readiness, and continuous performance improvement. Staff collaborate across disciplines to support organizational excellence and high-reliability healthcare. The ideal candidate will possess experience leading complex healthcare quality, regulatory, accreditation, patient safety, patient experience, or strategic improvement initiatives. They will be skilled in program management, data analysis, stakeholder engagement, regulatory compliance, and facilitating multidisciplinary teams to achieve organizational goals. When assigned to Contra Costa Health Plan: Two (2) positions will be assigned to the Quality and Health Equity unit. The first position plays a central role in advancing quality and health equity initiatives for CCHP. The coordinator oversees the Cultural & Linguistic Services Program, supervises the Health Education team, and takes lead on Health Equity initiatives. The role engages deeply with performance data analysis, stakeholder collaboration, and the development of culturally and linguistically appropriate services. Additional responsibilities include supporting the Community Advisory Committee, reviewing grievances related to discrimination or language access, advancing health equity strategies, preparing program evaluations and reports, and coordinating staff training initiatives to ensure compliance and continuous improvement across Medi-Cal, D-SNP, and commercial lines of business. The second position is an analytical position, focused on driving healthcare quality improvement efforts and supporting Population Health Management for CCHP. A core component of the role involves monitoring, analyzing, and interpreting key performance metrics, including HEDIS measures and D-SNP Star Ratings, to guide improvement strategies and ensure regulatory and contractual compliance. The position leads complex quality improvement projects, supports measurement year planning, and collaborates with internal and external partners to address gaps in care, and advances the Population Health Management Program while aligning quality initiatives with organizational goals, regulatory requirements, and member needs.
