Position at Danbury Hospital. The primary responsibility of this role is to patient safety and the strategic deployment of patient safety initiatives. This role plans, implements, and manages patient safety initiatives that decrease the risk of safety occurrences based on incident reporting and issues that arise through ongoing monitoring and reporting. It supports patient safety and quality improvement activities designed to identify opportunities and achieve improvements in patient care across the assigned department(s).

Requirements

  • Minimum formal education of Bachelor Degree, require.
  • Minimum 2 years experience in a clinical role.
  • Minimum of 3 years work related experience.
  • Registered Nurse (RN) license required.
  • BLS certification

Nice To Haves

  • Masters level degree or master's level prepared preferred.
  • APRN or PA license preferred.
  • CPHQ preferred.

Responsibilities

  • Ensures quality, patient safety and regulatory compliance for assigned clinical department(s).
  • Using process improvement methods, conducts analyses of data to promote increased efficiency, safety, revenue, satisfaction and quality.
  • Coordinates, implements, and evaluates patient safety initiatives and harm-reduction activities for assigned clinical departments. Recommends appropriate actions based on the analysis. Using project management skills, facilitates projects that have major organizational impact.
  • Coordinates all aspects of the Peer Review process and partners with Peer Review Committee Chairs to ensure appropriate case referral and thorough review of patient care.
  • Functions as an expert in the interpretation of Joint Commission, state and federal standards, and ensures adherence with requirements.
  • Promotes safety programs through presentation, staff education and education materials.
  • Collaborates with the Patient Safety/Risk Management office to identify risk management issues and opportunities for improvement.
  • Conducts Risk Assessments, Failure Mode and Effects Analyses & Root Cause Analyses to identify ways to reduce threats to patient safety & organizational exposure & works with the identified departments to implement corrective action that minimizes risks.
  • Responsible for patient relation activities as set forth by Site Director of Quality.
  • Conducts research and works with outside agencies on focused research initiatives.
  • Maintains confidentiality of information encountered through job activities.
  • Fulfills all compliance responsibilities related to the position.
  • Performs other duties as assigned.
