Quality Improvement Project Manager - ACO & Population Health RN
About The Position
This role is for a Quality Improvement Project Manager focusing on ACO and Population Health, acting as a liaison between Vigilance Health and its partners. The individual will be responsible for leveraging population health strategies, ACO quality measures, and quality improvement execution. Key responsibilities include using ACG to stratify patient populations, identifying opportunities for outcome and score improvement, and collaborating with partner QI teams and internal clinical leadership to implement changes. The role also involves being the ACG subject matter expert, explaining patient stratification to stakeholders, and identifying opportunities for relationship growth, such as expanding patient panels and launching new services.
Requirements
- Active RN license
- Subject matter expertise in ACO quality measures and population health quality improvement strategies
- Experience with Johns Hopkins ACG platform or a comparable risk-stratification / population health analytics platform
- Proven track record of building trust and driving results with external partners
- Comfortable holding your own with CMOs and clinical leadership
- Strong data instincts — comfortable digging into data sets to find stories and opportunities
- 5+ years of clinical experience
- Meaningful time spent in population health, quality improvement, or ACO operations
Nice To Haves
- CPHQ or a population health certification
- Experience evangelizing a technology platform or leading change management with clinical or partner audiences
- Experience growing a partner relationship — expanding patient panels or launching new service lines
Responsibilities
- Partner Relationship Management: Build and maintain relationships with partner QI teams and Vigilance-facing representatives, serving as the primary point of contact.
- ACG & Population Stratification: Analyze data within ACG and the Vigilance Care Portal to understand patient populations and identify improvement opportunities.
- Quality Measure Reporting & Analytics: Generate and maintain baseline reports on ACO quality measures for partners and the organization.
- Quality Improvement Strategy & Execution: Develop data-driven strategies for partners and targeted populations, and work with the Clinical Director to implement changes in workflows, training, and interventions.
- Business Development & Program Expansion: Identify new services, drive agreement renewals, and expand patient panels managed by Vigilance.
