About The Position

The Quality Assurance (QA) Nurse plays a crucial role in monitoring and improving clinical outcomes within a post-acute healthcare setting. This position focuses on ensuring compliance with quality measures, overseeing documentation, managing high-risk residents, supporting wound care programs, reviewing hospital transfers, and maintaining infection control standards. The QA Nurse collaborates with the Interdisciplinary Team (IDT) to identify trends, implement interventions, and ensure regulatory compliance. This role is essential for maintaining operational readiness and continuity of care across all shifts, ultimately aiming to improve resident outcomes in areas such as falls, wounds, quality measures, documentation, and hospital transfers.

Responsibilities

  • Monitor and trend CMS Quality Measures, identify residents at risk for QM triggers, ensure interventions are implemented and effective, collaborate with IDT to close gaps and improve outcomes, and prepare data for QAPI and leadership review.
  • Perform daily audits of CNA Point of Care (POC) charting in PCC, ensuring documentation is complete, timely, and reflective of care provided, validating alignment with care plans and resident condition, and providing real-time feedback and escalating concerns.
  • Review all falls for root cause and intervention accuracy, ensure timely implementation and follow-up of interventions, audit 72-hour documentation and IDT review, and maintain oversight of high-risk residents.
  • Audit skin documentation and weekly wound assessments, ensure treatments and physician orders are followed, validate completion of skin checks and admission assessments, and monitor pressure injury prevention practices.
  • Review all hospital transfers for appropriateness and trends, identify opportunities for early intervention, and ensure documentation supports clinical decision-making.
  • Conduct audits for hand hygiene, PPE use, and isolation compliance, support infection control processes and initiatives, and ensure adherence to facility policy and regulatory standards.
  • Monitor and support specialty clinical programs, including Non-Invasive Ventilation (NIV), by identifying eligible residents, verifying orders, ensuring compliance with usage parameters, auditing documentation and assessments, and supporting staff competency.
  • Review new admissions for order accuracy and completeness, ensure early alignment of documentation and care plans, and identify and address clinical risks.
  • Ensure compliance with CMS and state regulations, participate in audits, mock surveys, and plan of correction follow-up, and maintain readiness across clinical systems.
  • Collaborate with the Interdisciplinary Team (IDT) to address clinical findings, communicate trends, risks, and required follow-up, and support coordination of care across disciplines.
  • Provide guidance and support to nursing staff and assist with floor coverage as needed for safe operations.
