About The Position

Our Supported Housing program is looking for a Quality Assurance Coordinator to join the team. This position has the option of a hybrid schedule. This role will work closely with our Supported Housing team and program managers to ensure quality of care to those living with mental illness and funding compliance.

Requirements

• Bachelor’s Degree in health, human, or educational services and three years qualifying experience or equivalent preferred.
• Extensive experience can substitute for a degree.
• Experience working with people living with mental illness and/or other at risk populations required.
• Valid and clean NYS driver license required.
• Must have excellent organizational and time management skills.
• Prior experience working with in EHR systems and other databases preferred.

Responsibilities

• Work closely with the Supported Housing team to maintain quality care for clients.
• Assist program managers and staff for those living with mental illness and co-occurring disorders.
• Data tracking and quality review within EHR and funding database.
• Program documentation assistance.
• Tracking of Eligibility Determinations.
• Assist with grant/funding reports.
• Ensure all compliance effort are met for funding.
• Carry a caseload.
• Promote an environment of inclusion and belonging.
