About The Position

The Quality Assurance Coordinator Registered Nurse (QA RN) is responsible for leading, coordinating, and supporting the organization’s Quality Assurance and Performance Improvement (QAPI) program. This role ensures compliance with all applicable federal, state, and local regulations, including CMS and Department of Health requirements, while promoting a culture of safety, accountability, and continuous improvement. The QA Coordinator, RN serves as a clinical and regulatory expert, partnering with nursing leadership and interdisciplinary teams to monitor care processes, analyze outcomes, identify risks, and implement sustainable best practice improvements that enhance resident outcomes and maintain ongoing survey readiness.

Requirements

  • Current, unencumbered Registered Nurse (RN) license in good standing.
  • Minimum of 3–5 years of nursing experience, preferably in a long-term care or skilled nursing environment.
  • Demonstrated experience in quality assurance, regulatory compliance, or performance improvement.
  • Strong working knowledge of CMS regulations, the survey process, and QAPI standards.
  • Strong analytical, data management, and problem-solving skills.
  • Excellent written, verbal, and interpersonal communication abilities.
  • Ability to collaborate effectively with interdisciplinary teams.
  • Detail-oriented with strong organizational and time-management skills.
  • Proficiency with electronic health records (EHR) and quality reporting tools.
  • Dependable and punctual; reports to work on time and works scheduled shifts unless on approved PTO.
  • Able and willing to work required hours consistent with the needs of the position.
  • Maintains a professional appearance and demonstrates a courteous, service-oriented demeanor.

Responsibilities

  • Coordinate and maintain a comprehensive QAPI program in accordance with 42 CFR §483.75.
  • Develop and monitor facility-wide quality indicators, dashboards, and performance metrics.
  • Conduct routine, focused, and for-cause clinical audits, including but not limited to: Medication administration and storage, Falls and accident investigations, Pressure injury prevention and wound care processes, Documentation accuracy, completeness, and timeliness, Care plan compliance and effectiveness.
  • Analyze data to identify trends, performance gaps, and opportunities for improvement.
  • Lead root cause analyses (RCA) and investigations following adverse events, incidents, or survey findings.
  • Develop, implement, and monitor corrective and preventive action plans (Plans of Correction).
  • Chair or actively participate in Quality Assurance Committee meetings and performance improvement projects.
  • Ensure continuous compliance with CMS, state, and federal regulations governing long-term care.
  • Coordinate ongoing survey readiness activities, including mock surveys and internal tracers.
  • Review survey results and collaborate with leadership on timely and effective corrective actions.
  • Maintain required regulatory documentation, audit tools, and quality reports.
  • Proactively identify compliance risks and support departments in remediation efforts.
  • Provide education, coaching, and guidance to nursing staff and leaders related to quality standards, documentation practices, and regulatory expectations.
  • Support department leaders in developing and implementing performance improvement initiatives.
  • Serve as a quality and regulatory resource to nurse managers and interdisciplinary team members.
  • Utilize best practice data reporting systems, technology, and AI strategies.
  • Collect, track, and trend quality metrics, including: Falls, Pressure injuries, Medication errors, Hospital readmissions.
  • Prepare routine and ad hoc quality reports for leadership, committees, and governing bodies.
  • Support Quality Measure (QM) monitoring and performance improvement initiatives.
