About The Position

Southeastern Integrated Care (SEIC) is seeking a full-time Qualified Professional (QP) to support their Assertive Community Treatment (ACT) team in Wilmington, NC, and the surrounding area. The QP is a vital role on the SEIC enhanced services team, providing community-based care and case management to ensure clients receive high-quality care in their home or community location. SEIC's mission is to improve the lives they serve with comprehensive integrated health services, not bound by convention.

Requirements

• Must meet the requirements of a NC Qualified Mental Health Professional
• Minimum of a bachelor’s degree and 2 years of experience with the population served.
• Must possess a current, valid, NC driver’s license and insurance with a clean driving record.
• If offered a position, all offers of employment are contingent upon the successful completion of an extensive background check, motor vehicle records check, drug screening and required education verification as well as licensure or certification credentialing with our team.

Responsibilities

• Coordinating care and advocating for clients
• Updating PCP’s and entering documentation into the system
• Participate in developing PCPs
• Ensuring the quality care and safety of the individuals served
