About The Position
EPIC Health Partners is seeking a compassionate and motivated professional to join our Mental Health Skill Building team. This position plays a vital role in promoting recovery, independence, and wellness—all under the supervision of a licensed mental health professional. We welcome applications from all qualified QMHP, QMHP-T, and QPPMH candidates.
Requirements
- Registered as a QMHP, QMHP-T with the VA Board of Counseling, or certified as a QPPMH
- Bachelor’s degree or higher in a Human Services-related field (e.g., Psychology, Social Work, Counseling).
- At least 1 year of behavioral health experience.
- Valid driver’s license and reliable transportation.
- Current CPR/First Aid certification and TB screening.
Responsibilities
- Deliver person-centered care in homes and community settings.
- Assist in the creation and ongoing revision of service plans tailored to the needs of each individual.
- Monitor and ensure that service plans are being effectively carried out.
- Provide linkage to resources and case management support for individuals.
- Submit necessary documentation according to established timeframes.
- Maintain continuous communication with individuals and key stakeholders to ensure coordinated planning and service delivery.
- Participate in required training and team meetings.
