About The Position

Are you a passionate Mental Health Professional interested in working with the most vulnerable? We’re seeking a dynamic & innovative QMHP/T for our expanding Partial Hospitalization Program and Intensive Outpatient programs in Hampton, VA. In this role, you will be responsible for Group Facilitation in our PHP & IOP programs to help them achieve a better life. Requirements

• Bachelors degree in Psychology, Sociology, Human Services or related field
• Must have experience facilitating groups
• Must qualify as a Qualified Mental Health Professional (QMHP) or in training through the Virginia Department of Health Professions

Responsibilities

• Develop and maintain Individual Service Plans and Progress Notes on group participants.
• Work with individuals to foster new or improved skills on a group level.
• Case management functions of linking, arranging, and advocating for services and referrals.
• Maintain up-to-date individual records and prepare required session notes in a timely and accurate manner.
• Establish a good working relationship with other public and private agencies/services and interface collaboratively with them.
• Ongoing assessment of treatment needs of individuals.
• Maintain continuous contact with the identified individual and other key stakeholders in planning and service delivery.
• Maintain up-to-date individual records and prepare required/necessary reports in a timely and accurate manner.
• Establish a good working relationship with other public and private agencies/services and interface collaboratively with them.
