About The Position

The Qualified Medication Aide (QMA) will be responsible for observing and reporting resident reactions to medications, administering prescribed medications, and performing various patient care tasks under the supervision of a licensed nurse. This role requires adherence to strict protocols for medication administration, documentation, and resident safety. The QMA will also assist with other patient care duties as needed and maintain confidentiality.

Requirements

Must be a Qualified Medication Aide (QMA).
Must have completed a minimum of 6 hours of in-service training per year, including Medication administration, finger stick blood glucose testing, Hemoccult testing, & Medication administration via G-tube/J-tube.
Must be trained to administer specific medications.
Must be able to personally prepare (set up) the medication to be administered.
Must be able to document in the Clinical record all medications personally administered.
Must be able to obtain authorization from the licensed nurse on duty for PRN medication administration.
Must follow scope of practice when administering PRN meds.
Must be able to apply topical medications to minor skin conditions.
Must be able to administer medication via metered dose inhaler.
Must be able to conduct hemoccult testing.
Must be able to conduct finger stick blood glucose testing.
Must be able to apply a dressing to a minor skin tear that has been assessed by a licensed nurse.
Must be able to provide ordered site care to a healed G-tube or J-tube site.
Must be able to empty & change colostomy bag.
Must be able to instill a commercially prepared disposable enema.
Must be able to administer a sitz bath.
Must be able to apply cold, dry compress.
Must be able to collect fecal or urine specimens.
Must be able to document QMA observations in the Clinical Records/Progress Notes.
Must be able to prepare thickened liquids as ordered.
Must be able to assist licensed nurse in giving shift report.
Must maintain confidentiality of all pertinent resident care information.
Must be able to perform duties of CNA when assigned by the supervisor.
Must report any malfunctioning equipment.

Responsibilities

Observe and report any reactions and side effects to medications exhibited by a resident to the facility’s licensed nurse.
Measure and document vital signs prior to medication administration and report abnormalities to the licensed nurse.
Administer regularly prescribed medications that the QMA has been trained to administer, after personally preparing them.
Document in the Clinical record all medications that the QMA personally administered.
Initiate oxygen per nasal cannula or non-sealing mask only in an emergency, and immediately notify and document the action and notification to the licensed nurse.
Obtain oxygen saturation utilizing an oximeter and report results to a licensed nurse.
Apply physician-ordered oral, ophthalmic, otic, nasal, vaginal, and rectal medications.
Crush and administer medications if appropriate per manufacturer’s instructions or physician orders.
Count, administer, and document controlled substances.
Administer medications per G-tube or J-tube.
Administer previously ordered PRN medication only if authorization is obtained from the licensed nurse on duty and follow scope of practice.
Apply topical medications to minor skin conditions.
Administer medication via metered dose inhaler.
Conduct hemoccult testing and report results to the licensed nurse.
Conduct finger stick blood glucose testing and report the results to the licensed nurse.
Apply a dressing to a minor skin tear that has been assessed by a licensed nurse.
Provide ordered site care to a healed G-tube or J-tube site.
Empty and change colostomy bag.
Instill a commercially prepared disposable enema after assessment by a licensed nurse and instruction from the licensed nurse.
Administer a sitz bath, if ordered by the physician, and report any unusual observations to the licensed nurse.
Apply cold, dry compress as directed by the physician or by the licensed nurse in emergency situations.
Collect fecal or urine specimens as ordered by physician.
Document QMA observations, including what the QMA sees, hears or smells, and what is reported by the resident, in the Clinical Records/Progress Notes.
Obtain a minimum of 6 hours of in-service training per year, including Medication administration, finger stick blood glucose testing, Hemoccult testing, and Medication administration via G-tube/J-tube.
Prepare thickened liquids as ordered.
Assist licensed nurse in giving shift report.
Maintain confidentiality of all pertinent resident care information to assure resident rights are protected.
Perform duties of CNA when assigned by the supervisor.
Report any malfunctioning equipment.
Perform other duties as assigned by the supervisor.
