About The Position

The Qualified Intellectual Disability Professional / QIDP is responsible to assist and ensure the overall operations and services are being delivered according to state and federal regulations, agency policy and procedure and according to the agency’s mission. The role of the Qualified Intellectual Disability Professional / QIDP is responsible to assist in the overall coordinating and managing the overall person-centered services for assigned individuals. The Qualified Individual Disability Professional (QIDP) serves to provide leadership, direction, and training in order to supervise the overall development of a specific program. QIDP also supervises the rehabilitation of persons with developmental disabilities. Utilizing the Interdisciplinary Team approach, develop implementation strategies which outline services and supports for individuals so they may work towards the outcomes detailed in their Personal Plan. The QIDP supervises the treatment process in the assigned residences, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. The QIDP is responsible for ensuring service provision by all disciplines represented on the treatment team. The QIDP meets routinely with other Team Clinicians for coordination and integration of services. The QIDP coordinates with the Program Manager(s) and staff. The QIDP monitors provision of individual and group program services as well as the maintenance of a therapeutic milieu which is conductive to residents' health and well-being. The QIDP ensures provision of integrated services to individuals by fostering within each Team an awareness of the need for service integration; Coordinating service delivery and planning activities with day program services; and Monitoring service provision by agency clinical staff, consultants and community-based vendors, ensuring services are in accordance with IDT treatment recommendations. The QIDP ensures provision of a therapeutic environment through regular review of employee work schedules and performance, initiating corrective actions when necessary to ensure provision of coverage levels consistent with the treatment needs of the individuals and regular monitoring of the residential setting to ensure maintenance of an atmosphere conducive to individuals' development and well-being, where the rights of each individual are upheld and respected. The QIDP regularly records progress of individuals, and documents all changes in Plans of Care / ISPs according to procedures established by the agency.

Requirements

Bachelor's Degree in Human Services
At least 2 to 3 years of supervisory/managerial experience, preferably with the developmentally disabled in an OPWDD setting
Good communication skills (verbal and written) and ability to read, write, speak and understand English
Excellent interpersonal skills necessary to interact with coworkers, employees, residents and their families; along with an attention to detail
Must be experienced handling confidential information in the strictest manner
Proficiency in the use of computer and software programs
Ability to analyze problems and determine corrective measures
Goal-oriented professional with the ability to work independently, as well as part of a team, in a fast-paced environment.
A compassionate and caring communication skill for participants and their families combined with strong documentation, analytical and problem-solving skills are a must.
Excellent computer skills with knowledge of Microsoft Word and Excel required.
Ability to effectively collaborate

Responsibilities

Work as a team and collaboratively with other agency providers, departments, and administration.
Effectively communicate and support the organization’s mission, vision, and values, while monitoring and ensuring activities, both inside and outside of the Residential Support, comply with State and Federal regulations and Agency and Program specific policies and procedures.
Maintaining program records and ensuring compliance ongoing, which include but not limited to Life Plans, Staff Actions, IPOP, Assessments, Level of Care and DDP2. In addition to ensuring these plans and records are consistent with other clinical service plans, such as PONS and BSP.
Evaluation of individual’s strengths needs and develop Habilitation Plans, designed to develop personal and life skills.
Participate in the development of Life Plan in relations to the residential services.
Develop and supervise the implementation of the Staff Action Plan (SAP)/Residential Habilitation Plan (RHP) goals and the Individualized Protective Oversight Plan (IPOP)
Monitor the progress of individuals and write the monthly progress notes
Ensure the timely receipt and filing of the Interdisciplinary Team summaries/ reports from various professionals
Ensure timely completion of QIDP summaries for monthly, semi-annual and annual reviews.
Submit semi-annual and annual reviews to Care Managers for inclusion in the Life Plan.
The QIDP ensures timely completion and submission of required IDT documentation to the Residential Director.
The QIDP accepts primary responsibility for development of program goals and methodologies and for training DSPs to correctly implement and document such activities.
The QIDP may direct team members / clinicians to formulate program activities but remains responsible for ensuring that these activities are developmentally appropriate and are in response to identified prioritized needs.
Attend Life Plan (Individualized Service Plan) meetings (Annual review and 6-month review), monthly staff meetings, and Interdisciplinary Team (IDT) meetings
Complete addendum/changes to Residential Habilitation Plans as per team agreement.
Maintain contact with the individual’s family, Care Manager, Behavior Intervention Specialists, Nurses, Day Program, treatment centers, and other agency programs to ensure continuity of services.
Review and update annually and as needed, the IPOP, Assessment and other plans according to the individual’s needs, strengths, and necessary supports.
Ensure timely completion of all annual evaluations/ assessments by various professionals.
Participate in the internal audit process.
Review deficiencies with IDT and follow-up with appropriate parties.
Ensure that all Life Plans, Staff Actions, Monthly Notes, Daily Service Documentation data complies according to OPWDD and OMIG requirements.
Notify team members, parents, guardians, relatives and day programs of all team meetings, reviews and apprise them of all habilitation changes affecting individuals.
Participates in the Staff Development Program, providing training to subordinates to increase competencies and foster a team approach to service delivery and provides specific instruction to Residence Managers and Assistant Managers to enable them to develop programs, including (but not limited to) ADL, ILS, Recreational & Social Programs to meet individuals' needs, and for IRAs habilitation activities based on each individual’s 'valued outcomes.
Provides training to Program Management and DSPs in program implementation and documentation and supervises the development and implementation of each resident's Plan of Care / Life Plan in accordance with regulations.
Provide training to staff on Residential Habilitation Plans to ensure the appropriate implementation of plans.
For new individuals, works with the Intake Department and completion of a required documentation for compliant intake procedures
Manage and arrange admissions and/or discharges and will work closely with intake, program staff, family members, and agency representatives in effectively coordinating these processes.
Ensure admissions and discharge procedures are followed according to state regulations and agency policy; along with the team, review documentation for admissions and partake in Intake Meetings.
Assist with residential service development and increase bed-capacity.
Regularly monitors implementation of Plans of Care, initiating revision when: Current Goals / Objectives have been achieved; A Goal / Objective / Res. Hab. is determined to be inappropriate due to a change in the individual’s status, functioning level or interests; changes are required to meet regulations; (or) the individual or another team member requests such review/revision to better meet identified needs.
Coordinates and ensures the overall development, periodic evaluation and review of each individual's individual plan of care and supervises the delivery of training and rehabilitative services relative to this plan of care.
Reports regularly to the Supervisor on issues of program compliance, noncompliance, and remedial actions taken to correct deficiencies and identifies needs and requests supports, assistance or direction to meet service needs and ensure program compliance.
Participates in clinical reviews, incident review and human rights committee meetings and other meetings as directed by Supervisor.
As schedule allows, the QIDP is the primary chair on behalf of the agency in the Life Plan Meetings and reviews documentation and provides technical assistance or direction to program management and staff.
Coordinates monthly and/or semi-annual progress of each individual relative to the established program goals and objectives, and will document all progress, lack of progress or regression in measurable terms.
Work closely with professional consultants to see that all evaluations are appropriately scheduled, are performed on a timely basis, and focus on the client's previous progress, realistic potential, and the facility's approved curriculum.
Oversee and track staff Program Specific Training for staff compliance and that staff are adequately trained to implement individual full care plan.
Will remain familiar with each individual's total program, current status, and family or agency interaction as it pertains to each individual.
Ensuring goals are met in areas including client and family satisfaction, safety, quality, and team member performance.
Ensure and monitors all required reports, incident reports, corrective action, statistics, and quality assurance functions to verify program services are in accordance with agency and departmental Quality Assurance, Corporate Compliance and HIPAA policies and procedures.
Conduct compliance reviews of the program to ensure state and agency record and program compliance for all programs and directs and ensure delivery of services with Residential Habilitation Services.
Monitor and ensure the development and implementation of Person-Centered Plans, timely completion of Person-Centered Reviews.
Conduct Morning, Evening, Meal, and Medication Observation to ensure adequate supervision and that all safeguards and plans are followed and follow the Program Structured Procedures and ensure the Residential Habilitation program is adequately staffed, based on regulatory ratios; as well as implements residential schedules and procedures to ensure safety and organization.
Utilizes effective interpersonal, counseling and negotiation skills to handle sensitive issues with families, other providers, care managers, personnel, and other and external agency personnel.
When necessary, maintains on-going contact with the service providers, oversight offices, parents, advocates, care managers and other professions to ensure the continuity of service delivery.
Monitors and follow up to ensure the planning and coordinating services designed to: teach individuals life skills necessary to establish and maintain their independence and increase individuals’ participation in social and community-based activities, increase individuals’ range of socially appropriate behaviors and habits to increase acceptance and integration in the community at large and assist individuals in maintaining health and safety skills.
Attend residential services management meetings and agency wide management meetings, in addition to conducting management meeting on a regularly scheduled basis and as needed to disseminate information pertinent to the operation of the programs and agency.
Maintain a uniform record keeping system that ensures consistency, confidentiality, and company standards.
Responsible for the enforcement of Company Policies and Procedures and support the mission and vision of the organization.
Performs other related duties as requested by supervisor.
