About The Position

We’re looking for a passionate and driven QIDP who brings energy, compassion, and strong clinical skills to support individuals with intellectual and developmental disabilities through person-centered care.

Requirements

  • Bachelor’s degree in Human Services, Social Work, Psychology, Special Education, Nursing, or related field (meeting OPWDD QIDP requirements)
  • Minimum of 2 years’ experience supporting individuals with intellectual/developmental disabilities
  • Experience training and coaching staff on person-centered plans and goal implementation
  • Strong clinical writing skills (assessments, plans, progress notes)
  • Proficiency with MS Office and general computer use
  • Flexibility to work some early mornings, evenings, and weekends as needed
  • Valid NY/NJ/CT driver’s license and reliable transportation to travel between sites

Responsibilities

  • Partner with people supported using a strengths-based, rights-focused approach to understand their goals, interests, and needs
  • Lead the development and ongoing management of Person-Centered Plans (PCPs) that truly reflect each individual’s voice
  • Facilitate interdisciplinary team meetings and coordinate circle of support participation to ensure continuity of care
  • Develop clear, measurable goals and track progress—adjusting plans as needs and preferences evolve
  • Promote independence and community inclusion through meaningful activities, skill-building, and access to resources
  • Train and coach staff on plan implementation, documentation, and person-centered practices
  • Observe services in action, provide real-time feedback, and model best practices in dignity, respect, and inclusion
  • Coordinate annual and semi-annual planning meetings, ensuring full participation from individuals, families, and advocates
  • Maintain accurate, timely, and compliant clinical documentation
  • Serve as a liaison across residential, clinical, medical, and community services to ensure seamless care
  • Support individuals and families through transitions, challenges, and crises
  • Participate in quality assurance activities, audits, and continuous improvement efforts
