About The Position

As a Personal Care Partner II QMAP, you'll provide compassionate care to residents while maintaining the highest standards of medication administration. You'll master the "5 Rights" of medication—right resident, right dose, right time, right medication, and right route—while anticipating resident needs and ensuring their safety and well-being. Requirements

  • Per the state of operation requirements, Medication certification; QMAP-A/L, Med Tech, Med Aide, Nurse Delegation
  • Must be able to read, write and speak the English language.
  • Strong self-management and organization skills
  • Strong multi-tasking abilities with the ability to handle competing deadlines, flexible and adaptable (Open to change).
  • Ability to effectively handle difficult and sensitive issues and maintain confidentiality

Nice To Haves

  • One (1) year experience in some level of elder care preferred
  • Proficient computer skills preferred

Responsibilities

  • Assist residents with personal care needs and activities of daily living (ADLs), including bathing, dressing, grooming, and meal assistance.
  • Administer medications in accordance with state law and facility protocols, maintaining accurate records and alerting supervisors to any concerns.
  • Monitor and document resident health conditions, reporting changes or concerns to the Director promptly.
  • Anticipate and identify resident care needs through careful observation and communication with the care team.
  • Report any suspicions of abuse, neglect, or resource misappropriation in accordance with facility policy.
  • Maintain confidentiality and handle sensitive information with professionalism.
  • Restock medication carts and storage areas as needed.
  • Attend staff meetings and complete other duties as assigned.
