About The Position

The Caregiver/QMAP is responsible for providing personal services to assigned patients. Provide direct patient care, collaborate with the other Interdisciplinary Team (IDT) members within the care team, and with other health care providers in various care settings.

Requirements

  • High school diploma or GED
  • 1 year of caregiving experience
  • CPR and First Aid certification required, or must be obtained within 90 days of hire

Nice To Haves

  • QMAP highly preferred, but not required to start
  • QMAP must be obtained within 90 days of hire (we will reimburse exam costs)

Responsibilities
