We're looking for an organized, self-motivated, and enthusiastic Group Home Manager to join our team. The ideal candidate is responsible for the overall operations conducted at the Children’s Group Home that ensures appropriate services to the children in accordance with Department of Children and Family Services (DCFS) standards and the Department of Human Services (DHS).

Requirements

  • Bachelor's in Psychology, Social Work, or other Human Services fields.
  • Minimum age 25 with valid IL driver’s license and auto insurance required.
  • Reliable transportation and good driving record (No more than 2 moving violations in the last 5 years.
  • Must be able to show proof of automobile insurance and be an eligible DCFS driver.
  • Must be able to complete background checks and physical, drug test, and TB test.
  • At least 1 year of experience working with individuals with developmental disabilities.

Responsibilities

  • The QIDP is responsible for the overall coordination, monitoring, documentation and follow up on all programs, services, and plans for the individual’s assigned based on DHS regulation and LFI/Residential Standards and Best Practice.
  • Must be capable of reading, writing, and speaking English to plan, coordinate, and schedule programming with program staff and children, guardians/caregivers (when necessary), other agency staff, and stakeholders. Written communication must be clear, concise, grammatically correct, and free of spelling errors.
  • Must possess sufficient strength, agility, and sensory acuity to support program participants safely and effectively during programming hours. This includes responding to behavioral problems using agency approved safety training in which the individual is displaying aggression towards themselves or others, or other emergencies using learned safety measures, with adults weighing 150+ lbs. Must also assisting with daily living tasks such as toileting and dressing and be able to lift 50 lbs.
  • Must behave in a professional manner that reflects positively on themself and Little Friends Inc. This includes demonstrating high commitment to customer service and customer satisfaction for all internal and external customers of Little Friends Inc.
  • Must possess sufficient computer skills to plan for both in-house and community programming.
  • Must be aware of, and follow, the policies and procedures of Little Friends Inc. and Residential services.
  • Is required to complete all training and re-certifications per Little Friends, Residential, and DHS.
  • Shall lead or attend individual Implementation Strategy or Personal Plan meetings and CST’s as required.
  • Attend house meetings as scheduled.
  • Shall be responsible for the overall monitoring of individual programming including but not limited to, goals, choice making and progress for each individual. Problems in these areas should be discussed with the Director of Program Services as soon as possible. Individual programming shall be based upon the Personal Plan and individual needs.
  • QIDP will ensure all individuals assigned will have an Implementation Strategy whose strategies, as decided by the Personal Plan and team, meet program licensure and accreditation standards. The Strategies are developed, implemented, and reviewed monthly to ensure updates are completed and individual progress on objectives monitored. This information will be maintained in each individual’s record by the QIDP. Shall also provide training and resource support to house staff.
  • Shall present any potential Human Rights Restrictions to the appropriate committees and keep accurate and detailed records for the rights restrictions per DHS regulations and standards.
  • QIDP will attend and actively participate in audits. This includes supplying requested documentation, talking with auditors, taking auditors to homes etc.
  • Ensure the Settings Rules are being followed and provide any and all information requested for settings validations, updates, check-ins etc.
  • Shall provide training and resource support to the Case manager’s, Community Support Specialists’ and Support Staff in areas including but not limited to, training on individual Strategies and behavior plans.
  • Shall complete ICAPs annually or as requested.
  • Shall ensure that all client records are maintained in accordance to DHS.
  • Completes the QIDP Monthly report and submits the report to the Director of Program Services.
  • Shall be available via Cell Phone Monday at 9:00am through Friday at 5:00pm and maintain on-call responsibilities within established policy and rotation. When caring for someone else’s phone, will be responsible for all duties assigned to that QIDP or CM.
  • Shall attend all in-service training sessions and maintain Continuing Education Units (CEU’s) in accordance with state requirements. (12 CEU’s required annually)
  • Shall attend regular staff meetings to address individual clinical issues in the homes.
  • Shall participate in committees/teams as assigned.
  • Shall be responsible for other duties as assigned by the Director of Program Services which are consistent with the job duties and responsibilities.
  • Will submit weekly schedules for review and approval.
