About The Position
CardioRenal Vision is an established, physician-led post-acute practice specializing in Pulmonology, Cardiology, and Nephrology care for patients in skilled nursing and long-term care settings. They are seeking a dedicated Pulmonologist to join their growing team in the San Antonio region on a part-time basis. This role offers the opportunity to provide meaningful specialty care to a medically complex and underserved patient population while working within a collaborative, team-based environment.
Requirements
- Board Certified or Board Eligible in Pulmonology

- Experience in post-acute care settings
- Experience with skilled nursing and long-term care patients
- Strong communication and collaboration skills
Responsibilities
- Provide clinical oversight and collaboration with experienced Nurse Practitioners caring for sub-acute and long-term care patients
- Conduct onsite visits within skilled nursing and long-term care facilities
- Support development of individualized pulmonary care plans
- Participate in a physician-led, multidisciplinary care model focused on improving outcomes and reducing hospitalizations
