About The Position
Brookhaven Health Care Facility is seeking an experienced Pulmonary Registered Nurse (RN) to oversee and coordinate our Pulmonary Program. In this role, you'll provide clinical leadership, coordinate pulmonary-focused care, and collaborate with the interdisciplinary team to support residents with respiratory and pulmonary needs. Requirements
- Active New York State Registered Nurse (RN) license required
- Skilled nursing, rehabilitation, pulmonary, respiratory, or subacute experience preferred
- Strong clinical assessment and care coordination skills
- Experience with care planning and interdisciplinary collaboration preferred
- Strong leadership, organizational, and communication skills
- Commitment to providing high-quality, resident-centered care
Responsibilities
- Oversee and coordinate daily operations of the Pulmonary Program
- Coordinate individualized care plans for residents with pulmonary and respiratory needs
- Monitor resident conditions and provide advanced clinical support
- Collaborate with physicians, respiratory providers, therapy, nursing, and other interdisciplinary team members
- Assist with admissions, assessments, and care coordination for pulmonary residents
- Provide staff education on pulmonary care and clinical best practices
- Ensure compliance with clinical, regulatory, and facility standards
- Participate in quality improvement and Pulmonary Program development initiatives
