Requirements

  • Associate degree in nursing from an accredited college or university
  • One (1) year of nursing experience
  • Completion of a PHN course within one year of hire
  • Current Georgia Registered Professional Nurse license
  • CPR certification

Nice To Haves

  • One (1) year of experience at the lower-level PH Registered Nurse 1 (HCP190) or position equivalent.

Responsibilities

  • Identify and address the health needs of high-risk populations by developing and implementing targeted interventions based on population needs and available resources.
  • Participate in the planning, implementation, and evaluation of quality improvement initiatives.
  • Provide treatment, administer medications and vaccines, and deliver preventive and maintenance care to patients and families in accordance with care plans and/or physician treatment plans.
  • Screen and refer clients to appropriate community service providers.
  • Participate in drills and exercises related to pandemic influenza, mass sheltering, and mass dispensing.
  • Participate in the planning, organization, and evaluation of program areas to ensure program goals and objectives are met.
  • Provide clinical services to target populations.
  • Apply population health principles to promote health, prevent disease, and improve health outcomes.
  • Provide healthcare, first aid, immunizations, and assistance with convalescence and rehabilitation.
  • Conduct comprehensive assessments of clients’ healthcare needs.
  • Conduct community needs assessments.
  • Develop comprehensive plans of care for individuals, families, and populations.
  • Modify client treatment plans based on client responses and changing health conditions.
  • Provide health education, instructions, and classes to individuals and families to promote and maintain health.
  • Coordinate program services with healthcare providers, community organizations, and other service partners to support effective implementation.
  • Develop and sustain community partnerships.
  • Perform additional duties as assigned.
