About The Position

This position is funded by a grant for a duration of time as defined by the grantor. It coordinates the daily operations of the TennCare Presumptive Eligibility (TPE), Tennessee Breast and Cervical Cancer Screening Program (TBCSP), and Women’s Health Navigation Expansion activities. The role provides enrollment assistance, patient navigation, case management, provider coordination, data management, education, and community outreach.

Requirements

  • Two (2) years of experience in health promotion or health education capacity
  • Bachelor’s degree from an accredited college or university in public administration, health administration, business administration, social work, or closely related field; OR An equivalent combination of related education and/or experience.
  • Knowledge of care coordination and navigational activities.
  • Knowledge of medical terms and procedures, specifically related to Breast and Cervical diagnoses and treatment plans.
  • Knowledge of public administrative functions, programs, policies, and procedures.
  • Ability to work as a team member with diverse populations.
  • Ability to resolve issues and interpret policies, maintain confidentiality, prioritize and work independently while performing multiple tasks.
  • Self-directed with ability to work in a fast-paced, professional, diverse environment with minimal supervision.
  • Excellent oral, written, and interpersonal skills.
  • Demonstrated attention to detail.
  • Proficient in Microsoft Office (Word, Excel, and PowerPoint) and County application software.
  • Must possess a valid Tennessee driver's license or secure one upon date of employment.
  • Must have daily access to an operable automobile and meet county requirements for automobile insurance upon date of employment.
  • Must be willing to carry a cell phone for contact.

  • Experience coordinating and working on outreach and community activities is preferred.

Responsibilities

  • Enters and maintains program enrollment, service, transmittal, and quality-improvement data in designated databases.
  • Provides case management and patient navigation, including assessments, referrals, service authorizations, appointment scheduling, barrier identification, education, and follow-up.
  • Coordinates care with clients, primary care providers, specialists, clinics, and community partners; obtains and maintains required medical reports.
  • Supports TBCSP activities at participating health departments, federally qualified health centers, and organizational clinics.
  • Assists with training and monitoring participating sites and referral providers for compliance with program policies and grant requirements.
  • Conducts evidence-based education, outreach, and engagement activities focused on disproportionately affected populations.
  • Builds partnerships with screening sites, healthcare providers, schools, universities, hospitals, and community organizations.
  • Serves as a liaison between the Tennessee Department of Health Central Office and participating local sites.
  • Obtains and maintains Certified Application Counselor certification to assist eligible clients with enrollment.
  • Attends required grant conferences, trainings, coalition meetings, and work groups.
  • Assists during public health emergencies and participates in agency training(s) as deemed appropriate by leadership.
  • Performs other related duties as assigned or directed.
