About The Position

This position is responsible for providing public health nursing services to individuals, families, and population groups in Health Department clinics, client homes, and in the community, especially in regard to reproductive and sexual health and wellness. Nursing activities include but are not limited to: systematic analysis of health data; health education and advocacy; coalition building; and collaborating with community partners to promote the health of a population. This position will strive to: build healthy communities through promotion of healthy environments; achieve health equity and social justice; improve quality of life and increase access to care. This position may be involved with special health projects in the community including, but not limited to, the Community Health Assessment, nursing interventions, and goal setting with a client and family.

Requirements

  • ADN and 2 years of nursing experience
  • Registered Nursing license in the state of Ohio
  • Strong computer skills
  • Knowledge of modern medical procedures and protocols
  • Ability to work evenings & weekends during busy season
  • Must have and maintain throughout employment a valid driver’s license, reliable transportation, and be insurable under the Lucas County Commissioners Risk Management.

Responsibilities

  • Performs initial client assessment by acquiring health and family histories, conducts routine medical tests, and makes appropriate referrals.
  • Prepares and maintains client records through the entry of accurate and appropriate patient information into the Health Department’s electronic health records system.
  • Helps clients to obtain appropriate medical and social assistance; teaches and counsels clients and families; acts as an advocate and promote family autonomy.
  • Draws labs and completes tests as needed. Enters lab and test results into electronic health records; Follows-up with patient results as needed.
  • Ensures that the clinic is stocked with supplies. Checks inventories to ensure that supplies are not expired and safe to use with patients.
  • Cleans clinic area once patients are seen.
  • Audits charts to ensure that the best nursing practices are being adhered to.
  • Performs outreach functions, attends interagency meetings as assigned, and works collaboratively with all community agencies.
  • Counsels clients and provides health education in areas of reproductive health & wellness, health and medical problems and services, and life cycle tasks for the purpose of improving or enhancing the personal health of the client(s).
  • Adheres to all Federal, State and Local laws, regulations, rules and/or guidelines including HIPAA regulations as well as all grant requirements, where applicable.
  • Evaluates outcomes of public health nursing interventions; works with others (clients and other professionals) to make changes as necessary.
  • Mentors students as public health leaders to strengthen the public health nursing workforce.
  • Promotes evidence-based practice, maintains best practices, and determines and maintains essential Public Health Nurse Competencies.
  • Assists and works with other nursing staff as needed. This includes, but is not limited to, covering other nursing areas such as Adult Travel Immunizations, Shots4Tots, etc.
  • Trains in and covers other nursing competencies, programs, and units to maintain nursing competencies, ensure coverage, and respond to emergency and disaster situations.
  • Adheres to PHAB and Quad Council Coalition nursing core competencies including, but not limited to: 1A1, 1A4, 1A7, 1A10, 1A11, 2A4a, 2A8, 3A2, 3A6, 4A3, 5A1a, 6A1, 6A3, and 8A1.
  • Participates in quality improvement efforts and achieving PHAB accreditation requirements. This position will be responsible for using quality improvement (QI) processes and/or techniques to improve the effectiveness of the assigned public health program.
  • Attends and participates in conferences, seminars, trainings, or related education classes as assigned.
