About The Position

The Public Health Nurse (Per Diem) position with VNA of Cape Cod is responsible for ensuring compliance with the Nurse Practice Act and Standards of Practice in Massachusetts. This role involves functioning safely and competently within their scope of practice, participating in emergency preparedness, and serving as a liaison with local health agents for communicable disease surveillance and immunization planning. The nurse will provide resources and education on infectious diseases and emergency preparedness, manage supplies for contracted health programs, and promote immunization and wellness through community outreach. Additionally, the role includes providing health education lectures, primary nursing care as a Public Health Nurse to contracted departments, conducting safety assessments, managing medications, and communicating with physicians, patients, and families regarding health matters. The nurse will also be involved in ordering supplies, participating in the Infection Control Committee, and utilizing monitoring results to identify education needs and improve patient care. On-call availability is required, along with fulfilling communicable disease responsibilities in compliance with MDPH requirements.

Requirements

  • Successful passage of VNA pre-hire medication test
  • Registered Nurse currently licensed in the State of Massachusetts.
  • CPR required.
  • Minimum of three (3) years clinical experience.

Responsibilities

  • Assures compliance with Nurse Practice Act and Standards of Practice in the State of Massachusetts.
  • Functions within the parameters of knowledge and experience, practicing safely and competently.
  • Participate in emergency preparedness teams on the local and state level as assigned.
  • Liaison with the health agents in the town to coordinate communicable disease surveillance and initiate immunization plans, providing and/or coordinating emergency call coverage as indicated.
  • Provide resource and education to towns and agency in infectious disease and emergency preparedness planning.
  • Performs clinical functions and manages supplies for contracted health programs including clinics.
  • Promotes immunization and wellness programs through community outreach activities.
  • Provide health education lectures to community based organizations with in the VNA service/contract area.
  • Provide primary nursing as the Public Health nurse to contracted Public Health departments.
  • Complete safety assessments on at risk individuals and well-being checks.
  • Medications management and referral to appropriate sources for further assessment/assistance.
  • Communicate with physicians and counsel patient and family members as appropriate in relationship to communicable disease, infection control, wellness education and prevention guidelines.
  • With the Public Health Manager order all equipment/supplies appropriate and necessary for clinics.
  • Participates on agency Infection Control Committee as directed by the Public Health Wellness Manager.
  • Utilize results of monitoring and evaluation activities to identify education needs, improved patient care practices, and community outreach. Make training recommendations to HR Education Manager.
  • Provide on call availability in response to agency and community need.
  • Perform communicable disease responsibilities in compliance with MDPH requirements including reporting, surveillance and case management per town contracts.
