The Public Health Nurse (PHN) delivers comprehensive nursing services that promote health and well-being for residents of Marion County across various settings, including homes, schools, workplaces, clinics, and community sites. The PHN conducts assessment, education, case management, care coordination, immunization, communicable disease services (including TB), and surveillance activities; documents care in the electronic medical record (EMR); and collaborates with interdisciplinary partners to advance equitable health outcomes. The PHN represents Community-Based Care and works across multiple MCPHD departments to assure continuity of care for diverse populations. This position reports to the Northeast District Health Office. Your day blends field and clinic work. You will review referrals, conduct home visits to clients, administer immunizations in clinic, and provide TB case management services (DOT/DOPT coordination). You will document in the EMR, coordinate services with community partners, participate in outreach events, and respond to urgent public health needs. Throughout the day, you communicate clearly with families and multidisciplinary teams, uphold HIPAA, and advance health equity.

Requirements

Bachelor's degree in nursing (BSN) from an NLN-accredited program.
10 years of prior nursing experience
Typing and computer proficiency (≥30 WPM) and EMR documentation experience
Current Indiana Registered Nurse (RN) license
Current CPR certification
Skilled in independent nursing assessment, triage, care planning, motivational interviewing, and culturally responsive education.
Immunization practice (vaccine administration, storage/handling), TB control (case management, DOT/DOPT), communicable disease basics, maternal/child health.
Demonstrated commitment to health equity; ability to communicate effectively across languages/cultures and build trust with diverse communities.
Professional collaboration with agency/community partners; diplomacy and strong customer service.
HIPAA, confidentiality, and informed consent; accurate, timely EMR documentation; data collection for program monitoring and reporting.
Prioritization, problem-solving, and sound clinical judgment in dynamic field/clinic settings; ability to function independently and within teams.

Responsibilities

Interview clients/families; obtain histories; develop nursing care plans; make referrals; and ensure continuity via home visitation and follow-up.
Provide culturally responsive health education/counseling to individuals and groups on prevention, chronic disease, maternal/child health, and wellness.
Deliver comprehensive case management using a multidisciplinary approach; initiate referrals and coordinate resources across MCPHD and community agencies.
Provide care coordination for high-risk maternal/child clients; collaborate with Community Health Workers; educate on parenting, nutrition, prenatal/postpartum care, breastfeeding, safety, and substance use.
Conduct postpartum and newborn assessments during home visits; monitor newborn growth/development and support families.
Deliver TB services: case finding, medication coordination, therapy adherence monitoring, patient education, provider communication, and collaboration with TB epidemiology; oversee Community Health Worker activities for directly observed therapy.
Maintain clear, timely EMR documentation; manage a varied caseload (prenatal, infant/child, communicable disease, TB, refugee, chronic disease); protect confidentiality per HIPAA; work effectively within the team.
Perform diagnostic procedures, treatments, injections, and medication administration per provider orders and MCPHD protocols/policies.
Staff Nurse-of-the-Day (NOD) services (walk-in screening, counseling, referrals, vaccines, TB testing/medication follow-up).
Administer vaccines safely/accurately in immunization clinics; maintain cold-chain and documentation standards.
Support additional clinics (Refugee); collect vitals/specimens, document histories, and perform related procedures.
Collaborate with MCPHD programs and external agencies (e.g., school services, health fairs, communicable disease events, emergency preparedness).
Participate in public health emergency preparedness exercises and responses as directed; report for specialized duties as required.
Mentor nursing students and precept new nursing staff.
Develop/assist with health education programs informed by community feedback.
Serve as a consultant to schools; attend multidisciplinary case conferences; represent MCPHD at community activities.
Attend team/staff/committee meetings; complete mandated trainings (HIPAA, BBP, Emergency Preparedness).
Participate in in-services/seminars; may facilitate orientations and trainings.
Participate in a minimum of two H.O.P.E. (health fair) events totaling at least eight (8) hours annually.
Protect and maintain confidentiality of client/employee information; secure records when transporting to other settings or home visits.
Complete National Incident Management System (NIMS) IS-100 and IS-700.
In a public health emergency, availability for weekends or alternate schedules is required.
