About The Position

This position is primarily responsible for managing the pediatric and adolescent immunization program, educating parents, patients and community partners on vaccine-preventable diseases, administering vaccines under federal (VFC/317) and state guidelines, and coordinating off-site community clinics. This position is also responsible for managing CMH client cases. This position will provide vital clinical preventive services and cross-program support (including Adult Travel, Tuberculosis Control, and Reproductive Health Nursing activities include but are not limited to systematic analysis of health data; health case management; health education and advocacy; coalition building; and collaborating with community partners to promote the health of a population. This position will strive to build healthy communities through promotion of healthy environments; achieve health equity and social justice; improve quality of life and increase access to care. This position may be involved with special health projects in the community including, but not limited to, the Community Health Assessment, nursing interventions, and goal setting with a client and family.
