This position is currently grant-funded through per-member-per month payments tied to contracts between Medicaid, insurance companies, and local health departments. At this time, funding is secured through December 31, 2026. Join our team as the Lead Nurse Coordinator for the Care Management for At-Risk Children (CMARC) program, serving children ages 0–5 in Alamance County who are at increased risk for developmental delays or health challenges due to prematurity, congenital conditions, toxic stress, or complex family environments. In this leadership role, you will provide comprehensive, family-centered care management services to eligible children and families while guiding the daily work of two CMARC Social Workers through mentoring, caseload assignment, and annual performance evaluations. Working closely with the Care Management Nursing Supervisor and the multidisciplinary Care Management Team, you will help shape program operations, strengthen community partnerships, and promote an optimal level of functioning for each child by ensuring high-quality, coordinated, family-centered care. The CMARC program coordinates medical, nutritional, educational, social, and emotional services to ensure children receive the support needed for a healthy start. Care Managers work closely with private providers and community partners to strengthen family stability and promote long-term well-being.
