Public Health Nurse - Family Health
About The Position
Carver County is seeking a Public Health Nurse (PHN) to promote and protect the health of its residents by delivering essential public health services. This role is ideal for someone driven to improve public health outcomes and values a career with purpose, stability, and growth. The PHN will work under the direction of the Family Health Supervisor, utilizing the Foundational Public Health Services Framework. This position combines clinical expertise with systems-level thinking to address health equity, prevent disease, and improve population health. The PHN will provide direct services and systems-level interventions in areas such as infectious disease control, immunizations, tuberculosis case management, maternal/child health, refugee health, emergency preparedness and response, and resource navigation. Upon successful completion of the training period, this position is eligible for partial remote work. This is a safety-sensitive position.
Requirements
- Registered Nurse's License with Public Health Nurse Certification in the State of Minnesota
- Two years paid work experience as a four year RN and/or PHN
- Basic Cardiac Life Support Certification
- Valid Minnesota driver's license or Equivalent combination of education and experience necessary to perform the essential functions of the position
Nice To Haves
- Experience in maternal/child health, infectious disease, emergency preparedness and response, working with families and children with special health conditions and/or adverse childhood experiences, and history working with evidence-based home visiting models (e.g., MECSH).
- Experience with contact investigation and contact tracing and/or other work related to infectious disease prevention, intervention, mitigation and recovery.
- Fluent in Spanish
Responsibilities
- Conduct case investigations and contact tracing for reportable diseases
- Provide education and guidance to affected individuals and communities
- Collaborate with healthcare providers, schools, and MDH to contain outbreaks
- Administer immunizations in clinics, schools, and community settings
- Educate the public on vaccine-preventable diseases and immunization schedules
- Coordinate screening, treatment, and follow-up for active and latent TB cases
- Monitor treatment adherence and side effects
- Serve as liaison between clients, providers, and public health labs
- Implement evidence-based family home visiting models to support healthy pregnancy outcomes, child development, and caregiver well-being
- Promote and facilitate participation in the Follow Along Program and refer to early intervention systems (e.g., Help Me Grow)
- Connect individuals and families to housing, food, mental health, and social services
- Advocate for culturally responsive care and equitable access
- Identify service gaps and contribute to policy and program development
- Assess community health needs using data and informatics
- Design and evaluate evidence-based interventions
- Collaborate with schools, healthcare, community partners, and public health colleagues
- Participate in emergency preparedness and response activities
