About The Position

This position is currently grant-funded through per-member-per month payments tied to contracts between Medicaid, insurance companies, and local health departments. At this time, funding is secured through December 31, 2026. Join a mission-driven team making a real impact in Alamance County. We’re seeking a dedicated public health nurse to provide care management and program coordination for the CMHRP program. In this role, you’ll support individuals and families through clinic work, home visits, and community partnerships—helping improve health outcomes where it matters most. You’ll use strong clinical judgment, public health knowledge, and care management skills as part of a multidisciplinary team. Work is guided by the NC Nurse Practice Act and evidence-based public health standards. This position is ideal for a nurse who values autonomy, meaningful relationship-building, and improving population health. As part of Public Health, you may also assist in emergency preparedness efforts such as vaccination clinics, shelters, or medication distribution. Requirements

Graduation from an accredited school of professional nursing
One to two years of professional nursing experience
Demonstration of leadership experience such as charge nurse, preceptor, committee chair, or direct supervision of staff.
Current licensure as a Registered Nurse in North Carolina by the North Carolina Board of Nursing
A valid North Carolina driver's license (or must be obtained within 60 days of relocating to North Carolina)

Responsibilities

Provide comprehensive CMHRP Care Management services for pregnant Medicaid members who are at risk for poor birth outcomes.
Assess, plan, coordinate, and evaluate care while offering support to meet the medical, preventive, and social needs of pregnant and postpartum women.
Connect patients with medical, nutritional, educational, social, and emotional resources to promote healthy pregnancies and positive birth outcomes.
Conduct clinic, community, physician office, and home visits to meet program requirements.
Serve as part of a multidisciplinary Care Management Team, providing leadership for medically complex patients and acting as a nursing resource for non-nurse team members.
Work toward program goals established by the State Medicaid program and prepaid health plans.
Utilize multiple electronic medical record systems.
Review and report program data.
Assist with staff onboarding and training.
Support student learners.
Provide direct supervision for 3.5 multidisciplinary team members.
Assist in emergency preparedness efforts such as vaccination clinics, shelters, or medication distribution.
