The Public Health Caseworker I (Journey Level) is assigned to a multi-disciplinary public health team and works to identify client needs, provide psychosocial support, and make referrals for basic needs and community resources. This position works collaboratively with other service organizations and community partners to support adults, children, youth, and families. Additionally, all Health Department employees participate in public health emergency response activities and perform public health services in accordance with public health standards and practices. The ideal candidate for the Public Health Caseworker I role is a compassionate professional dedicated to supporting Weld County families through an active, community-facing role. Fluency in both Spanish and English is required to build trust and deliver individualized care to a diverse population. Highly adaptable and self-motivated, this candidate excels at assessing holistic client needs, teaching healthy behaviors, and collaborating with a multi-disciplinary team. They divide their time evenly between providing specialized care management for the HCP program (Children and Youth with Special Health Care Needs) to navigate complex health systems and advocate for unique medical needs, and managing community outreach and clinic services, which include connecting individuals to resources and delivering health education, coaching, and care coordination. They work efficiently with minimal supervision, manage confidential electronic health records with strict attention to detail, and stand ready to assist with public health emergency response activities when needed.
