About The Position
The Physical Therapy Assistant will provide assistance in the treatment and rehabilitation of patients within the home health setting, performing responsibilities such as executing physical therapy plans and helping patients improve their mobility and physical function.
Requirements
- BLS certification (AHA required).
- Proficiency with Homecare Homebase (HCHB) charting system.
Nice To Haves
- Experience preferred, but new grads are considered.
Responsibilities
- Execute prescribed physical therapy plans under the supervision of a physical therapist.
- Conduct 'start of care', recertifications, evaluations, and standard patient visits.
- Maintain accurate and timely records of patient progress.
- Coordinate with the clinical manager or educator for support and guidance as needed.
